Tactical Ops and the New Rules of Combat Fire Engagement

This session will present the new rules of combat structural fire engagement and provide insights into integrated command and operational risk management, tactical safety and tactical protocols based on occupancy risks versus occupancy type. Building and occupancy profiling requires knowledge of emerging construction methods, features, systems and components. Coupled with the increasing commonality of extreme fire behavior and the increased fire load package, these factors require new skill sets in reading the building and implementing predictive occupancy profiling to determine appropriate tactics for firefighters, company and command officers.

The class will examine case studies, history-repeating events, the latest testing and research findings on vent path theory, fire behavior, structural system integrity, wind driven fire theory and fire suppression theory, and engage students through interactive exercises and group discussions.

Reading the Building: Predictive Occupancy Profiling

As we approach the July 4th holiday period, two significant LODD incidents previously occurred during this time frame that hold a number of lessons learned related to command management, operations, building construction principles and building performance, fire behavior and the ever present dangers of the job.

Twenty-Three Year Anniversary Hackensack Ford Fire and Truss roof collapse, Hackensack Fire Department. July 1st, 1988

Pause to remember our brothers who made the ultimate sacrifice twenty-three years ago, on July 1st, 1988 and the lessons learned from this event.

On July 1, 1988 Hackensack’s Captain RICHARD L. WILLIAMS, Lieutenant RICHARD REINHAGEN, Firefighter WILLIAM KREJSA, firefighter LEONARD RADUMSKI, and Firefighter STEPHEN ENNIS lost their lives at Hackensack Ford when a bowstring arch truss collapsed entrapping them in the area below. The five firefighters were in the structure, a bowstring truss building, when the roof suddenly collapsed a 60-foot square section of the building’s wood bowstring truss roof collapsed, and an intense fire immediately engulfed the area. Williams, Kresja and Radumski were killed instantly, and four other firefighters escaped. Reinhagen and Ennis survived the initial collapse and found refuge in a tool room where they spent the next 13 minutes calling for help.. . despite heroic rescue attempts, succumbed to carbon monoxide poisoning. Approximately 90 minutes after the collapse, firefighters located the bodies of their fallen comrades.

Three (3) building factors contributed to the collapse of this bowstring trussed roof:

• Alterations that consisted of a heavy ceiling of cementitious material on wire lathe;
• Auto parts storage in the attic; and
• The Fire burned for a significant length of time and was well advanced prior to detection.
• This roof collapsed 35 Minutes after the initial units arrived.

NFPA SUMMARY
Hackensack, New Jersey Fire Fighter Fatalities July 1, 1988

Five fire fighters from the Hackensack, New Jersey Fire Department were killed while they were engaged in interior fire suppression efforts at an automobile dealership when portions of the building’s wood bowstring truss roof suddenly collapsed. The incident occurred on Friday, July 1, 1988, at approximately 3:00 p.m., when the fire department began to receive the first of a series of telephone calls reporting “flames and smoke” coming from the roof of the Hackensack Ford Dealership.

Two engines, a ladder company, and a battalion chief responded to the first alarm assignment. The first arriving fire fighters observed a “heavy smoke condition” at the roof area of the building. Engine company crews investigated the source of the smoke inside the building while the truck company crew assessed conditions on the roof. For the next 20 minutes, the focus of the suppression effort was concentrated on these initial tactics.

During this time, however, little headway appeared to have been made by the initial suppression efforts, and the magnitude of the fire continued to grow. The overall fire ground tactics were shifted to a more “defensive” posture (exterior operation) and the battalion chief gave the order to “back your lines out.” However, before suppression crews could exit form the interior, a sudden partial collapse of the truss roof occurred, trapping six fire fighters. An intense fire immediately engulfed the area of the collapse. One trapped fire fighter was able to escape through an opening in the debris. The other five died as a result of the collapse. This incident and several others before and since, provide important lessons to the fire service regarding the fire ground hazards of wood truss roof assemblies.

Following is an excerpt from the New York Times article:
Demers contended that Chief Williams, primarily because of the volume of fire on the rooftop, should have ordered nine firefighters out of the garage within 7 minutes of his arrival. The order to pull out was given at 3:34 p.m., about 30 minutes after his arrival, the report said.

  • “This radio message was not acknowledged by any companies,” the report said.

The roof collapsed at 3:36 p.m. Three firefighters were hit by burning debris and killed, four escaped, and two, Lieut. Richard R. Reinhagen and Stephen Ennis, took refuge in the tool room.

  • At 3:39 p.m., Lieutenant Reinhagen began to radio his location and appeal for help, the report said.

In one of the major communications flaws cited by Mr. Demers at the fire scene, all departmental communications were transmitted on a single channel, or frequency. Consequently, Lieutenant Reinhagen’s appeals for help were intermingled with orders for deploying men and hoses and instructions to arriving companies.

  • “You have to hurry, we’re running out of air,” Lieutenant Reinhagen said at 3:42 p.m.

Headquarters then radioed to Chief Williams: “Expedite on that, they’re running out of air.” The transcript did not show any response from Chief Williams.Over the next 6 minutes, through 3:48 p.m., Lieutenant Reinhagen made 10 more calls. None was answered. For three of the minutes, bells indicating depletion of his air tanks’ supply were ringing repeatedly. At one point, a civilian who overheard the ringing on a radio scanner called fire headquarters to tell officials of the noise.

At 3:49 p.m., the Lieutenant radioed: “Chief, this is Lieutenant Reinhagen. I’m still stuck back in the right rear of the building in the closet. We are out of air in a closet. We’re out of air.”
“What’s your location?” Chief Williams said. The response was inaudible and the Chief began ordering water from a truck.

At 3:50 p.m., the Lieutenant got the Chief directly and repeated that they were “stuck in a closet” and “out of air.”

  • “Stuck in a closet?” Chief Williams asked.

Twelve seconds later, the Chief Williams asked: “Where you at?”

  • “Right there in the closet,” came the response.
  • Fourteen seconds later, Lieutenant Reinhagen radioed again: “Help. The right rear. Out of air. Anybody out there? Stuck in the closet, right rear. No air. Help.”

The Lieutenant was asked if he was on the first or second floor. “First floor, underneath the collapsed ceiling,” the Lieutenant said at 3:52 p.m. It was his last transmission. Firemen eventually punched a hole through an exterior wall about 10 feet from the tool room, but saw only a mass of flame, Mr. Demers said. The burning timbers were leaning against the tool room, he said, but neither fireman was burned.

Three Firefighters and Three Sisters Killed in Gloucester City, New Jersey Building Collapse during Fire Attack, Rescue Operation, July 4th, 2002

Gloucester City (NJ) Collapse 2002

On July 4th, 2002 at 0136 hrs.,The Gloucester City Fire Department was dispatched to 200 North Broadway for a reported house fire. Responding units were advised that occupants may be trapped. First arriving units were on location in less than three minutes.

They found heavy fire on all exposures of a three-story multi-family dwelling and initiated a search for entrapped occupants. (Various reports from bystanders were at times conflicting regarding the number and location of victims). While providing an aggressive interior attack and rescue operation, an occupant was rescued from the dwelling. Due to the severity of their injuries they were unable to give direction regarding the whereabouts of any other occupants.

While all hands were operating by continuing an aggressive interior attack and rescue, a partial collapse of the structure occurred. An emergency evacuation signal was sounded and while that was commencing a further and much more substantial collapse occurred trapping eight firefighters inside the burning debris.

Additional specialized collapse rescue resources were requested, firefighter accountability was initiated and rescue efforts were intensified. Five of the eight trapped firefighters were rescued. Three of the eight gave the ultimate sacrifice in service to their fellow man. Unfortunately these three children did not survive. A total of nine victims were transported to area hospitals, one civilian and eight firefighters.

NIOSH REPORT: Structural Collapse at Residential Fire Claims Lives of Two Volunteer Fire Chiefs and One Career Fire Fighter – New Jersey, HERE

SUMMARY
On July 4, 2002, a 30-year-old male volunteer fire chief, a 40-year-old male volunteer deputy fire chief, and a 30-year-old male career fire fighter died when a residential structure collapsed, trapping them, along with four fire fighters and an officer who survived. At 0136 hours, a combination fire department and a mutual-aid volunteer fire department were dispatched to a structure fire. Local law enforcement radioed Central Dispatch reporting a fully involved structure with three children trapped on the second floor. The first officer on the scene assumed incident command and reported to Central Dispatch that the incident site was a three-story structure with fire showing and that people could be seen at the windows. Note: The female resident (survivor) was the person seen in the window.

The three children that were reported as being trapped did not survive and were later found in the debris. Additional units were requested, including a mutual-aid ladder company from a career department. Crews were on the scene searching for occupants and fighting the fire for approximately 27 minutes when the building collapsed.

NIOSH investigators concluded that, to minimize the risk of similar incidents, fire departments should;
• Ensure that the department’s structural fire fighting standard operating guidelines (SOGs) are followed and refresher training is provided
• Ensure that the Incident Commander (IC) formulates and establishes a strategic plan for offensive and defensive operations
• Ensure that the incident commander (IC) continuously evaluates the risk versus gain during operations at an incident
• Ensure that a separate Incident Safety Officer, independent from the Incident Commander, is appointed
• Ensure that fire fighters conducting interior operations (e.g., search and rescue, initial attack, etc.) provide progress reports to the IC
• Ensure that accountability for all personnel at the fire scene is maintained
• Ensure that a Rapid Intervention Team (RIT) is established and in position
• Ensure that the officer in charge of an incident recognize factors (e.g., structural defects, large body of fire in an old structure, etc.) when analyzing potential building collapse
• Ensure, when feasible, that fire fighters should respond together, in one emergency vehicle, as a crew
Additionally, municipalities should consider
• Establishing and maintaining regional mutual-aid radio channels to coordinate and communicate activities involving units from multiple jurisdictions

In order to minimize the risk of similar incidents, the New Jersey Division of Fire Safety identified key issues that must be addressed and remedies that should be implemented within all departments.

1. FACTOR: There appears to be a disconnect between career and volunteer personnel in the Gloucester City Fire Department (GCFD). Many personnel expressed the concern that the GCFD operated as separate fire departments rather than as one.

REMEDY: It is essential that all firefighters put individual differences aside in order to work together successfully as a team to achieve their common goal of saving lives and property.

2. FACTOR: The GCFD, faces a common dilemma associated with combination fire departments: staffing levels may be unpredictable depending on how many volunteers are available to respond to any one incident. This unpredictability can result in insufficient staff to perform required tasks until additional staff arrives.

REMEDY: Elected or appointed municipal officials need to make a commitment to the adequate staffing of the fire department and staffing levels must allow for compliance with the two-in / two-out provisions of the Public Employees Occupational Safety and Health (PEOSH) Standard 29CFR1910.134. The New Jersey Division of Fire Safety can provide assistance to the municipalities and provide examples of how this can be accomplished

3. FACTOR: Due to the limited number of firefighting personnel who arrived at this incident, all initial efforts were focused on the rescue of occupants. This postponed fire suppression operations until additional resources arrived. Because rescue and fire suppression operations were performed sequentially rather than simultaneously, the fire may have spread more quickly resulting in the early failure of the structure.

REMEDY: Sufficient personnel are critical to ensure that all necessary operations can be performed at the appropriate time. Furthermore, a continual size-up assessment must be maintained so that the Incident Commander (IC) can be kept aware of the conditions as the incident progresses. This continual size-up will allow the IC to modify the strategy and / or tactics as deemed necessary.

4. FACTOR: Although the GCFD was equipped with a thermal imaging camera (TIC), firefighters failed to utilize it for the initial search for victims. The TIC was also not used properly to analyze the scope of the incident and determine what tactics to employ.

REMEDY: Fire departments that possess TIC units should use them regularly during routine operations such as training, scene size up, search and rescue and structural fire fighting.

5. FACTOR: From the onset of operations, the Incident Management System (IMS) was not properly expanded as the incident progressed. Given the scale of this incident, the span of control quickly became too large for the IC to effectively manage and additional functions were not delegated to subordinates. Critical tasks such as safety and accountability were not effectively implemented.

REMEDY: N.J.A.C. 5:75 mandates that all fire departments utilize an IMS. It is a modular system, which allows the IC to apply only those elements that are necessary at a particular incident, and allows elements to be activated or deactivated as incidents escalate or decline. Fire departments are required to adopt written plans, or Standard Operating Guidelines (SOG’s) based on the IMS, to address different types of incidents. The NJ Division of Fire Safety distributed suggested SOGs upon adoption of this regulation and they continue to be available to all fire departments.

6. FACTOR: The GCFD did not assign a dedicated safety officer (SO) to observe operations and terminate potentially unsafe actions.

REMEDY: IMS regulations under N.J.A.C. 5:75 mandate the use of safety officers (SO’s) at all incidents. An SO is required to observe operations on the fire scene, identify next steps and order the correction of safety hazards to personnel. Given the scope of this incident, the IC should have assigned at least one SO.

7. FACTOR: The GCFD did not designate accountability officers to monitor each area of entry into the structure. Nor was a Personal Accountability Report (PAR) or roll sheet utilized to track personnel and monitor their functions. Therefore, the concept of accountability of personnel location, function, and time failed.

REMEDY: Although not enforceable at the time of this incident, the regulations for the NJ Personal Accountability System (NJPAS) under N.J.A.C 5:75 now require that fire departments utilize an accountability system. This system includes the designation of accountability officers and the use of PAR’s / roll calls, all within the framework of the IMS that is required to be utilized at all incidents. The NJ Division of Fire Safety is in the process of finalizing suggested SOGs and will distribute them to all fire departments when complete.

8. FACTOR: Although firefighters Sylvester and Stewart were equipped with Personal Alert Safety System (PASS) devices, they did not activate them prior to entering the structure. It should be further noted that their PASS devices were not automated; they had to be manually activated by the user. Firefighter West was not equipped with a PASS device.

REMEDY: PASS devices must be provided, used, and maintained in accordance with PEOSH regulations under N.J.A.C. 12:100-10 et seq. Although many departments still rely on PASS devices that must be activated manually, – devices that are acceptable by PEOSH regulations – they are not ideal because the firefighter must remember to activate the PASS device. For this reason, fire departments should strongly consider upgrading their SCBA to those employing automatic activating PASS devices.

9. FACTOR: The GCFD did not specifically designate the required personnel for the rescue of distressed firefighters through the establishment of Rapid Intervention Teams (RIT) or Firefighter Assist and Search Teams (FAST). Consequently, when the building collapsed, there was not a properly equipped team in place for immediate rescue operations.

REMEDY: IMS regulations under N.J.A.C. 5:75 require that fire departments utilize RIT or FAST to rescue distressed firefighters when operating in a hazardous atmosphere. The IC should request a RIT or FAST as soon as possible after dispatch to allow the team to arrive quickly.

10. FACTOR: Not all fire departments operating on the fire ground were communicating on the same radio frequency, which resulted in communication failures. Although, the Camden Fire Department (CFD) did have the capability to communicate on the GCFD “Fire 5” frequency they chose not to.

REMEDY: IMS regulations under N.J.A.C. 5:75 require that a communication system allow for inter-agency communication during mutual aid responses by providing a direct communication link between companies. Fire departments should work with other departments that are used routinely for mutual aid to ensure radio interoperability.

11. FACTOR: An emergency evacuation signal was sounded upon reports of a firefighter missing inside the structure before the impending collapse, however, the signal was never sounded at any other time prior to the collapse, nor was it sounded immediately after the collapse.

REMEDY: In the event an emergency evacuation becomes necessary and an emergency signal is required, N.J.A.C. 5:75 requires that fire departments utilize an emergency evacuation signal that is easily recognizable and distinguishable from all other fireground noises. The signal must be utilized when conditions on the fireground indicate an imminent and extreme risk to firefighters. At this time NJ DFS is finalizing a proposal that would establish a statewide emergency evacuation signal.

12. FACTOR: During this incident, fireground conditions were not properly analyzed, which led to the failure to recognize an impending building collapse.

REMEDY: Firefighters and officers need to learn the warning signs and causes of building collapses. Often following a collapse, as was the case with this incident, personnel on the scene report that the structure collapsed “without warning”. However, this is usually not the case; the reality is that the IC and firefighters simply failed to identify the indicators that were present prior to the collapse.

13. FACTOR: After removal of all victims, the remaining structure was demolished and the incident scene was cleared of all debris within 48 hours of law enforcement concluding their origin and cause investigation. This prevented a thorough assessment of the remaining structure in order to identify the cause and contributing factors of the collapse.

REMEDY: A protocol should be adopted to ensure that fire scenes are secured in a manner that not only allows for public safety, but also prevents immediate demolition. This will provide agencies with an opportunity to conduct any investigations that may be necessary.

14. FACTOR It was difficult to gauge the amount of training for all GCFD personnel due to insufficient record keeping. Although it was determined that the GCFD firefighters and officers met the minimum regulatory training requirements, many members did not possess a great deal of supplemental training with regard to structural firefighting. Additionally, the volunteer firefighters and officers often did not attend the scheduled departmental drills and rarely trained with the career personnel despite having frequent opportunities to participate.

REMEDY: Standards such as NFPA 1500 recommend that fire departments establish a regular training and education program that is commensurate with the duties and functions that firefighters are expected to perform. Additionally, proper record keeping is essential to certify that all personnel have received both required and supplemental training or education.

15. FACTOR: Qualifications of volunteer officers were difficult to judge and there were serious concerns voiced by the career members of the department regarding the suitability of some of the volunteer officers. This resulted in a lack of confidence by several career personnel in the volunteer officers and reluctance to take direction from them.

REMEDY: In addition to the NJ DFS requirement that all fire service supervisors obtain incident management certification; municipal officials need to establish uniform minimum qualifications for fire officers in order to ensure the effective provision of fire suppression services to the public. The NJ DFS recently adopted voluntary fire officer standards and will be developing a training curriculum to meet those standards.

16. FACTOR: It was not possible to determine if a smoke detector inspection was conducted in the building after a change in occupancy in October of 2001 as required by the NJ Uniform Fire Code. The city’s housing department, who has the responsibility for these inspections, was unable to provide documentation of such an inspection to either the Division of Fire Safety or to the Camden County Prosecutor’s Office. It was not clear whether smoke detectors were activated during this fire incident.

There have been two fire ground collapse events this week; the first in Gary Indiana on April 5th, the other in Washington DC on April 8th that resulted in a total of eight firefighters being injured. The collapse conditions presented themselves during the course of operations in which suppression or search and rescue operations were being conducted.  Each occupancy and construction type presents unique challenges and risks related to construction, materials, dead load and resiliency when impacted by fire, heat or fire suppression activities.

Maintaining effective and focused situational awareness of developing and progressing fire conditions, position and company assignments, and related monitoring of occupancy risk profile conditions may provide timely insights to changing conditions that may influence the incident action plan, strategies and tactics deployed or implemented. 

As always, when physical conditions allow and there is an uncertainty of building risk profiles, occupancy charactoristics, construction type and fire conditions, a 360 is advised.

Never under estimate the severity of what may transpire when a partial collapse of a roof or ceiling assembly may have on operating companies and personnel. 

  • When ever feasible, timely opening up of concealed spaces within the ceiling void, cockloft or truss loft of a roof assembly is imperative to assess the extent of fire, travel and intensity.
  • Observations openings within the ceiling membrane (from below) or roof deck (above) allows assessment determination for impingement of structural or support members and systems.
  • Use caution and be conservative in the use of Thermal Imaging devices for determining extent and magnitude of fire conditions within the concealed compartment; Refer to test results from the UL Structural Stability of Engineered Lumber in Fire Conditions Report and test data, HERE
  • When feasible, ensure eitehr dedicated truck/ladder companies or assigned task resources are available to provide coordinated tactical support to interior suppression and search and rescue assignments to manage fire behavior factors with appropriate incident, occupancy and building defined tactical deployments.
  • Think about what’s burning above you…it may very well be burning around or ontop of you, if systems, assemblies or components fail.

Three Gary (IN) firefighters were injured when the third floor ceiling of a burning building collapsed on Tuesday April 5th during search and rescue operations. According to published reports their injuries weren’t believed to be life-threatening, but they were taken to a hospital.

Battalion Chief Robert Groszewski stated about 20 firefighters responded to the fire at the site of the former Campbell Friendship House. He says no one was at home when the fire began.

Groszewski says the fire may have begun in a third-floor stairwell.

The fire and collapse in Washington, DC has resulted in five DCFD firefghter injuries, of which one firefighter is in critical condition following a roof collaspse, entrappment and mayday in an unoccupied single family residential structure during primary search and rescue operations that was known to have homeless people occupy the structure on occassion.  According to various published reports, companies were making entry with pronounced fire conditions when the roof collapsed trapping the operating companies.

Reports from both STATter911.com and DCFD provided the following; DC Fire & EMS Department spokesman Pete Piringer indicated that five firefighters were hurt during a two-alarm house fire at 813 48th Street, NE. The fire was reported around 12:40 this morning. Three of the firefighters were from Rescue Squad 3 and were caught in the collapse of the roof of the one story, wood frame, single family home. At 7:30 AM Piringer reported one firefighter was in critical condition with significant burns, the other three with varying degrees of burns with expected early release.

OVERVIEW

Fifteen years ago, on March 18, 1996, two firefighters were killed in Chesapeake, Virginia when they became trapped by a rapidly spreading fire in an auto parts store and a pre-engineered wood truss roof assembly collapsed on them. The cause of the fire was an electrical short created when a power company truck working in the rear of the building drove away with its boom in an elevated position, accidentally pulling an electrical feed line from the main breaker panel at the rear of the store.

Post-incident investigations indicate that the electrical fault may have sparked multiple points of fire origin throughout the roof structure of the building, due to improperly grounded wiring. At the time of the report issuance, this was exemplified as another incident illustrating the rapid failure of lightweight construction systems when key support components are involved in a fire. The report pointed out the importance of prefire planning and accurate size up by fire companies to determine the risk factors associated with a fire in this type of construction.

Lessons regarding importance of initial company actions, constant re-evaluation of action plans, strong command and coordination of units on the fireground, and recognition of signs of impending structural failure were also reinforced.

Fifteen years later, reading through any number of NIOSH, USFA or NFPA reports, similar issues, challenges and operational factors resonate and continue to shape and challenge today’s fire ground operations.

It is without exception that the knowledge and insights being gained by the recent and past UL and NIST Research Studies coupled with the recommendations, from the NIOSH Fire Fighter Fatality Investigation and Prevention Program (HERE)

Today’s fire ground is changing at a very rapid pace as it relates to the continued evolution, transition of engineered structural components and systems (ESS). Are you prepared, knowledgeable and understand that new strategic and tactical approaches are required?

SUMMARY OF KEY ISSUES 

Staffing : The first alarm response provided a small attack force with limited capabilities. The full response brought only 10 personnel. 

Size-up : The first arriving company officer was not able to determine the location and extent of the hidden fire. 

Pre-fire plan information: This complex required a pre-fire plan due to the complex arrangement, multiple occupancies, mixed construction, lack of fixed protection, limited access and difficult water supply problems. The first-due company did carry a pre-fire plan that showed the layout of the shopping center and the floor plan for the auto parts store, but the prefire plan was not referenced by the crew during the fire. 

Delayed response: The first arriving company was on the scene alone for several minutes with only 3 personnel. The back-up companies had long response times. The lack of evidence of a working fire prompted the initial incident commander to return some of the responding units, resulting in even longer response times. 

Water supply: The first-in company did not establish a water supply. This required the second engine company to be committed to this task. 

Incident command: The battalion chief was faced with a complicated and rapidly changing situation. He was not able to effectively transfer command from the initial officer and direct the operations of widely separated units. 

Operational risk management:The officers involved in the initial part of the operation had to make critical risk management decisions with limited information. 

Accountability: Accountability for the personnel operating in the hazardous area was not established prior to the structural collapse. As the situation became critical, no one realized that a crew was still inside the building. 

Rapid intervention crew:  Additional crews did not arrive in time to assist the crew that was in trouble inside the building. 

Radio communications: The lack of a clear radio channel for fire ground communications caused serious problems with command and control of the incident, including the failure to maintain communications with the crew inside and the failure to hear their request for assistance. 

Lightweight construction: The roof collapsed quickly and with very little warning. This should be anticipated with a lightweight wood truss roof assembly. This hazard was not recognized by the crews on the scene. 

BUILDING DESCRIPTION - Construction and History 

The fire occurred in a modern, lightweight construction building that was added to an existing strip mall in 1984. The older mall on exposure side four was separated from the fire building by a masonry fire wall and was constructed with masonry walls and a steel bar-joist roof structure. The exposures on side two consisted of additional stores that were similar in construction to the auto parts store. There were no exposures on sides one and three. 

The auto parts store was constructed with two masonry exterior walls and two wood frame exterior walls, with a lightweight wood truss roof assembly. It was approximately 120 feet deep and 50 feet wide, providing about 6,000 square feet of open display and storage space. The roof assembly was a pre-engineered lightweight wood truss assembled from 2 x 6 top and bottom chords, with 2 x 4 web members held together with metal gusset plates. 

  • There were no interior bearing walls or supports for the roof structure. At one end, the trusses were supported by a wood plate that was bolted to a metal beam.
  • The other end rested on top of the concrete block wall. Each truss was separated by 24 inches and they were covered with 1/2 inch CDX plywood sheathing under a two-ply rubber membrane.
  • A drywall ceiling was attached to the underside of the trusses, creating a truss void space (truss loft) 24 to 36 inches above the ceiling.
  • A sheet rock divider was located in the middle of the truss void as a draft stop. The roof had a slight pitch.
  • Three air handling units were on the roof of the building, with an estimated combined weight of 3,000 pounds. It is not known when these units were installed and they may have represented an unanticipated dead load on the roof assembly.
  • There was no indication that the trusses had been reinforced to support the extra weight of these units.
  • The original truss roof structure collapsed during the construction of the building, injuring three workers.
  • Most of the trusses were damaged and had to be replaced at the time. The fire building was occupied by Advance Auto Parts, a chain distributor of automobile part and lubricants. The store was designed with an open retail area containing display racks for goods.
  • A long counter ran from front to back behind which was shelving for additional auto parts. Waste oil and batteries were kept in a rear storage area separated from the front of the store by a drywall wall.
  • The southwest corner of the building contained employee restrooms which had a small water heater located in the ceiling space just above them. The main entrance to the store was through two large glass doors at the front of the building. A delivery and service entrance was located in the rear and a 40 foot trailer was parked behind the building and used for additional storage.

THE FIRE 

At approximately 11:00 a.m. on March 18, 1996, a power company employee set up a service truck at the rear of the Indian River Shopping Center in Chesapeake, Virginia. The worker was going to disconnect the electrical power to a customer who had not paid an electrical bill. The customer, a cocktail lounge and bar, was located adjacent to Advance Auto Parts. In preparing to disconnect service, the power company worker elevated the articulating boom on his truck to roof level. Faced with the immediate loss of power, an employee of the lounge paid the electrical bill while the power company employee was beginning work, and went to the back of the store to show the receipt. 

A stamped receipt indicates the bill was paid at 11:16 a.m. at a supermarket also located in the shopping center. The power company employee, working from the bucket of the articulating boom, lowered the boom and verified the receipt. Although the bucket had been lowered, the hinged elbow of the articulating boom remained elevated. The employee then radioed his supervisor from the cab of his truck, and received instructions not to disconnect power. 

The power company employee then attempted to drive the service truck away, forgetting to secure the boom, which snagged on a power line feeding the meter at the rear of the Advance Auto Parts Store. This caused a phase-to-phase and phase-to-ground arcing fault at the store’s electrical meter, starting the fire. The power company employee immediately stopped, exited his truck, and cut the remaining power connections to the meter at the rear of Advance Auto Parts. 

Initial Actions Prior to Calling 911 

After cutting the power line to the building, the power company employee removed the meter, noticed smoke coming from the meter base, notified his office and requested that another power company crew and a supervisor come and assist him. 

  • An employee of the Advance Auto Parts Store came to the rear of the building and met the power company employee, telling him that the store had lost electrical power and that a fire was being extinguished inside the building.
  • Another Advance Auto Parts employee discharged a dry chemical fire extinguisher on the spot fire that had started near the hot water heater above the employee restrooms.
  • All believed the fire had been extinguished at this time.
  • At 11:29 a.m., the Chesapeake Fire and Police Emergency Operations Center received a 911 call from Advance Auto Parts reporting a problem with the fuse box in the store.
  • The Chesapeake Fire Department was dispatched to a report of a fuse box sparking at 4345 Indian River Road at the Advance Auto Parts store.

Emergency Response 

  • Initial response consisted of two engines, a ladder company, and a battalion chief, for a total of 10 personnel.
  • Engine 3 was the first due arriving company, responding from quarters. Engine 1 and Ladder 2 also responded.
  • Battalion 1 was dispatched as the command officer, but requested that Battalion 2 cover the assignment, since he was out of position.
  • Battalion 2 acknowledged the request, and he responded with the first alarm companies.
  • Engine 3’s crew consisted of three personnel: a driver/pump operator; Firefighter- Specialist John Hudgins, serving as Acting Lieutenant for the shift; and Firefighter- Specialist Frank Young, detailed to the station for the day, was riding in the jump seat. Engine 3 was responding in a reserve engine that had a 500 gallon water tank.

 

Initial Size-Up and Company Actions 

At approximately 11:35 a.m., about five and a half minutes after dispatch, Engine 3 arrived on the scene at the front of the strip mall. 

  • Hudgins reported “a single-story commercial structure, nothing showing from the front. Engine 3 is in command.”
  • Engine 3 took a position in front of the Advance Auto Parts Store. Hudgins and Young entered the structure from the front of the building to investigate.
  • Conditions were clear in the store, and there was no visible smoke or flames showing. They discovered light smoke near the electrical panel in the rear of the building, and radioed to Battalion 2 that they had a fire and were checking for extension.
  • Acting Lieutenant Hudgins then radioed for Engine 3’s driver to reposition the apparatus to the rear of the building.
  • Hudgins then radioed to Battalion 2, who had not yet arrived on the scene, that Engine 3 and Ladder 2 could handle the incident. Battalion 2 and Engine 1, the second due engine company, both went in service.

 Engine 3 Reports They Are Trapped, Roof Collapses 

At approximately 11:49 a.m., almost 20 minutes after the initial dispatch time, Hudgins radioed that he and Young could not get out of the building. Battalion 2 radioed back that he could not understand their transmission. Hudgins then radioed that they needed someone to come to the front of the building and get them out. Again unable to understand their transmission, Battalion 2 radioed for any unit on the fireground to advise him if they heard the message that was transmitted. 

  • Engine 4 responded that they were unable to copy the transmission.
  • Engine 14 then marked on the scene and was instructed by Battalion 2 to lay a supply line to the front of the building. Battalion 1, enroute to the fire on the second alarm, radioed to Battalion 2 that it sounded like someone was trapped inside.
  • Battalion 3, also enroute, radioed that he would be on the scene momentarily and would assist.

At this time, Ladder 2’s crew was setting the outriggers and preparing to elevate their aerial ladder for defensive operations. 

  • In the short time it took to accomplish the stabilization of the ladder truck, the front of the store became fully involved, the building contents ignited, and the roof collapsed.
  • Due to the radiant heat, Ladder 2 was forced to retract their outriggers and reposition to a safer defensive position on side one of the structure, and set up the aerial again.
  • Ladder 2’s crew did not hear Engine 3’s transmission that they were trapped.
  • Simultaneously, Engine 1 ran out of supply line about 200 feet short of the hydrant. Engine 2, responding on the second alarm, picked up the hydrant that Engine 1 was attempting to reach and laid a supply line to side one.
  • The driver of Engine 1 attempted to contact his officer by radio to advise that he could not reach the hydrant, but could not get through due to heavy radio traffic.
  • He parked the engine in the roadway, donned his SCBA, and went to the rear of the building to report to his Captain and rejoin his crew.
  • Battalion 3 arrived on side one about this time and radioed for all companies to switch to channel two, an alternate fireground tactical frequency.

Driven by the northerly wind and the draft created by the burning contents of the structure, the fire at the rear had grown in such intensity that personnel were forced to move Engine 3. Assisted by employees of the power company, Engine 3 was moved back away from the rear of the building. At 11:55 a.m., about 26 minutes after dispatch, the Captain of Engine 1, with his crew at the rear of the building, confirmed to Battalion 2 that “I got men on the inside from Engine 3, and the lines have been burned. I do not know their status, and we still have no water to go in after them.” 

Battalion 3 met with Battalion 2 and discussed that they may have lost a crew inside. Battalion 3 assumed command and Battalion 2 went to the rear of the building to coordinate rescue efforts. There, Battalion 2 met with the Captain from Engine 1. 

By this time, the building was fully involved and no rescue efforts could be mounted until the fire was knocked down. Officers at the front and the rear attempted to conduct a personnel accountability report (PAR) to determine who was missing and where they might be located. 

  • An engine company responding on mutual aid from the Virginia Beach Fire Department was flagged down, connected to Engine 1’s supply line, and completed the water supply to a hydrant behind the shopping center within the City of Virginia Beach. Engine 3 was forced to move back once again, and the supply line was disconnected from Engine 3 and used to supply water to Engine 4, a telesquirt that was positioned for defensive operations at the rear.

Extinguishment and Body Recovery 

The fire spread to the attic of the exposures on side two and was held in check by the fire wall on side four of the building. The fire was brought under control as the contents of the auto parts store burned off and several aerial streams were put into operation. After the fire was extinguished, a search for the missing firefighters was initiated. After the bodies of the firefighters were located, they were  removed from the fire building by members of the Virginia Beach Fire Department, and transferred by members of the Chesapeake Fire Department to medic units. 

The body recovery was supervised by the Chesapeake Fire Department Fire Marshal’s Office and documented. An investigation was immediately started by the Chesapeake Fire Department Fire Marshal. 

ANALYSIS 

Fire Cause and Flame Spread 

  • The fire was caused by the electrical short created when the power company truck struck the power line to the building. Investigation by the City of Chesapeake Electrical Inspector after the fire revealed that the meter contained wiring that appeared to have been tampered with and did not comply with the electrical code.
  • Several connections at the meter had been double-lugged, connecting multiple wires to single terminals. Additional investigation by Virginia Power revealed that the building may have been improperly grounded, leading to numerous hot connections when the short circuit occurred. The main fuse did not trip at the breaker panel and the wiring on all three air handling units had been fused. This probably resulted in the ignition of multiple spot fires in the truss loft above the store.
  • It appears that the fires in the truss loft were still relatively minor when Engine 3 arrived, but the fire spread rapidly throughout the space due to the light wood construction.
  • The wind drawn from the open doors at the front of the building also promoted rapid fire growth. This would have created a tremendous hidden fire in the wood truss loft area despite clear conditions inside the structure.
  • Reports of heavy smoke and fire conditions on the roof at the same time Engine 3’s crew was calling for pike poles and personnel to come inside are indications towards this scenario.
  • The interior of the auto parts store contained racks of auto parts and supplies, including oil, lubricants, rubber, and plastic parts. The contents were packed closely together and stored in tall racks near the ceiling.
  • Once the fire had broken through the ceiling in the rear of the building, these contents would have quickly reached their ignition temperatures, creating flashover conditions in the rear of the store as the fire progressed, trapping the firefighters and forcing them to seek an exit at the front of the store.

Roof Collapse 

  • The collapse of the pre-engineered truss roof occurred approximately 21 minutes after the time of dispatch, and within 35 minutes of the initial accident, that caused the electrical short.
  • The structure appears to have collapsed within 10 to 12 minutes after the truss space became heavily involved.
  • The collapse of similar truss assemblies under fire conditions within this time period has been well documented.
  • Post-incident investigations indicate that this truss assembly may have been weakened by deficiencies in the connection of the trusses to the beam on the east side of the building.
  • Also, the dead load of the three air conditioning units may have contributed to the rapid failure of the roof.
  • Reports from firefighters on the scene indicate that a partial failure of the truss assembly may have occurred in the rear of the building, followed shortly by the failure of the entire roof assembly.
  • It is possible that the crew of Engine 3 was trapped by the partial collapse of the roof in the rear, or by the collapse of racks containing auto parts in the building, or by the rapid spread of the fire and smoke which had broken through the ceiling.
  • It is also possible that a combination of these events occurred simultaneously. The failure of the entire roof assembly and complete involvement of the interior of the building with fire took place within one minute after the firefighters radioed for help, before any reaction to assist them could take place.

  

  

Fire Operations 

  

Initial Response - The first alarm assignment was overwhelmed by the situation, the circumstances, and the unusual sequence of events that occurred at this incident. It is evident that a larger force would have been needed to initiate an effective offensive or defensive operation for a working fire in a 6,000 square foot commercial occupancy, with attached exposures on two sides, with or without the unusual complications. 

  • The response of two engine companies, one ladder company and a battalion chief, provided a total of 25 only 10 personnel on the initial assignment.
  • The individual companies, which responded with three person crews, had limited capabilities to perform tasks independently.
  • This incident generated only a single call to 9-1-1 reporting an electrical problem.

  

 

LESSONS LEARNED AND REINFORCED  

1. RISK ASSESSMENT is the primary responsibility of the incident commander. 

This incident presented a very high risk to the firefighters who were attempting to make an interior attack. However, the risk factors were not recognized and the interior crew was not directed to abandon the building. Risk assessment should be a continual process, particularly when a situation is changing very quickly. 

2. ACCOUNTABILITY is an essential function of the Incident Command System. 

The location and operation of the initial attack crew was not tracked according to the incident command system that was in effect at the time of the fire. The system must keep track of the location, function, status, and assignment of every individual unit or company operating at the scene of an emergency incident. In order to be effective, the accountability process must be routinely initiated at the beginning of every incident and updated as the incident progresses and units are reassigned to different tasks. 

3. TACTICAL RADIO CHANNELS are essential for firefighter safety. 

The fireground operations were conducted on the same radio channel as the routine dispatch and transfer of additional units, hampering the fireground communications during the important early stages of the incident. Designated radio channels should be set aside specifically for communications between the incident commander and the units operating at the scene of an incident. The exchange of information, orders, instructions, warnings, and progress reports is essential to support safe and effective operations. Tactical channels should be assigned early and routinely to avoid the confusion that occurs when units that are already working are directed to switch to a different radio channel. 

4. FIRE OPERATIONS must be limited to those functions that can be performed safely with the number of personnel that are available at the scene of an incident. 

The initial response to this incident did not provide enough resources to safely initiate an effective interior attack for the situation that was encountered. The first arriving company initiated interior operations that could not be adequately performed or supported with the limited number of personnel at the scene or responding. The delayed arrival of back-up companies increased the risk exposure of the first due company. The situation called for a more conservative initial attack plan and/or an early retreat when the magnitude of the fire became evident. 

5. WATER SUPPLY is a critical component of a safe and successful operation. 

The failed attempt to establish an adequate and reliable water supply for the interior attack was a critical problem at this incident. This task occupied the second due engine company which was needed to provide either a back-up hose line to support the interior attack or a rapid intervention crew. 

6. LIGHTWEIGHT WOOD TRUSS CONSTRUCTION is prone to rapid failure under fire conditions. 

If the construction of the building had been known or recognized, the early failure of the roof structure should have been anticipated and the interior crew should have been withdrawn. This requires pre-fire planning to identify high risk properties and a reliable system to label the building or to inform the responding units of the risk factors of the building. It is usually difficult or impossible to make this determination when the building is burning.

Rules of Engagement Project; Increasing Firefighter Survival

Developed by the Safety, Health and Survival Section International Association of Fire Chiefs

The International Association of Fire Chiefs (IAFC) is committed to reducing firefighter fatalities and injuries.  As part of that effort the nearly 1,000 member Safety, Health and Survival Section of the IAFC has developed the recently approved  “Rules of Engagement of Structural Firefighting” to provide guidance to individual firefighters, and incident commanders, regarding risk and safety issues when operating on the fireground. The intent is to provide a set of “model procedures” for Rules of Engagement for Structural Firefighting to be made available by the IAFC to fire departments as a guide for their own standard operating procedure development.

In August, 2008, following a year of discussion, the Section moved to develop a set of “Rules of Engagement for Structure Firefighting”. A project team was created consisting of Section members and representatives of other several other interested fire service organizations. These included the Fire Department Safety Officer Association (FDSOA), the National Fallen Firefighter Foundation (NFFF), and the National Volunteer Fire Council (NVFC), the National Institute of Occupational Safety and Health (NIOSH) and other organizations. All draft material has also been shared with representatives of the International Association of Fire Fighters (IAFF) who developed a joint IAFF/IAFC “Fire Ground Survival Project”. Three Section members also participated in the IAFF project. The direction provided the project team by the Section leadership was to develop rules of engagement with the following conceptual points;

  • Rules should be a short, specific set of bullets
  • Rules should be easily taught and remembered
  • Rules should define critical risk issues
  • Rules should define “go” or “no‐go” situations

Early in development the Rules of Engagement, it was recognized that two separate rules were needed one set for the firefighter, and another set for the incident commander. Thus, the two sets of Rules of Engagement described in the attached document. The ROE were also inserted in the August issue of FireRescue magazine. Each set has several commonly shared bullets and objectives, but the explanations are described somewhat differently based on the level of responsibility (firefighter vs. incident commander).

The attached and linked PDF document reflects nearly two years of public comment and feedback from several presentations at fire service conferences, including the National Fallen Fire Fighters Safety Summit held at the National Fire Academy this past March. The “Rules” were formally adopted by the IAFC Health, Safety and Survival Section at the Fire Rescue International Conference held last week in Chicago.

The development of the rules integrated several nationally recognized programs and principles. They included risk assessment principles from NFPA Standards 1500 and 1561. Also included where concepts and principles from Crew Resource Management (available from iafc.org) and data and lessons from the National Near‐Miss Reporting System (firefighternearmiss.com). The development process also included review of lessons learned from numerous firefighter fatality investigations conducted by the National Institute of Occupational Safety and Health (NIOSH) Fire Fighter Fatality Investigation and Prevention Program.

It’s incumbent that the fire chief and the Departments management team insure the safety of all firefighters working at structural fires. All command organization officers are responsible for their own safety and the safety of all personnel working with them. All officers and members are responsible are responsible for continually identifying and reporting unsafe conditions or practices. The Rules of Engagement allows both the firefighter and the incident commander to apply and process these principles.

One principle applied in the Rules of Engagement is firefighters and the company officers are the members at most risk for injury or death. The Rules integrate the firefighter into the risk assessment decision making process. These members should be the ultimate decision maker as to whether it’s safe to proceed with assigned objectives. The “Rules” allow a process for that decision to be made while still maintain command unity and discipline. It is well known that firefighting is hazardous with varying levels of risk to the firefighter. However, firefighting is not a military campaign where lives are lost to establish a beach head. No firefighter’s life is a building that eventually will be rebuilt. Keep all members safe so “Everyone Goes Home”!

The Incident Commanders Rules of Engagement for Firefighter Safety

  • Rapidly Conduct, or Obtain, a 360 Degree Size‐Up of the Incident.
  • Determine the Occupant Survival Profile.
  • Conduct an Initial Risk Assessment and Implement a SAFE ACTION PLAN.
  • If You Do Not Have The Resources to Safely Support and Protect Firefighters – Seriously Consider a Defensive Strategy.
  • DO NOT Risk Firefighter Lives for Lives or Property That Can Not Be Saved – Seriously Consider a Defensive Strategy.
  • Extend LIMITED Risk to Protect SAVABLE Property.

As we approach the July 4th holiday period, two significant LODD incidents previously occurred during this time frame that hold a number of lessons learned related to command management, operations, building construction principles and building performance, fire behavior and the ever present dangers of the job. Take the opportunity to learn more about these events, and expand your insights and knowledge base.  Those events being the 1988 Hackensack (NJ) Ford Fire which resulted in five (5) LODD and the 2002 Gloucester City (NJ) Fire that resulted in three (3) LODD along with three children.

Buffalo, NY Three Alarm Fire and Double LODD Report

8-26-2009 7-07-53 AMNIOSH released it’s report on the August 24, 2009 three alarm fire at 1815 Genesee Street in Buffalo, New York that resulted in the LODD of Lt. Charles McCarthy and FF Jonathan Croom. On August 24, 2009, 45-year-old career Lieutenant Charles McCarthy died following a partial floor collapse into a basement fire, and  34-year-old career fire fighter Jonathan Croom was fatally injured while attempting to rescue the Lieutenant.  The Buffalo Fire Department was dispatched for “an alarm of fire” with reported civilian(s) entrapment. Arriving units discovered a heavily secured mixed commercial/residential structure with smoke showing. Following failed initial attempts to locate an entry to the basement, crews located a door on Side 2 that provided access down a flight of stairs to a basement entry door. Repeated attempts were made to force open this basement door in order to search for trapped civilians, but crews had difficulty gaining access through this door because it was made of steel and locked and dead-bolted on both sides. Other crews on scene performed primary searches of the 1st and 2nd floors with no civilians found.

Approximately 30 minutes into the basement fire, command ordered all interior crews to exit the structure to regroup because crews were still unable to gain access into the basement from Side 2. Additional manpower was sent with special tools to assist in breaching the basement door on Side 2. Lieutenant Charles McCarthy and two fire fighters from his crew entered into the structure from Side 1 to verify all fire fighters had exited a 1st floor deli. Lt. McCarthy following a hoseline into the structure, was well ahead of the other two fire fighters when the 1st floor partially collapsed beneath him. McCarthy fell with the floor into the basement, exposing him to the basement fire. The other two fire fighters immediately exited the deli after fire conditions quickly changed and shelving and displays fell on them; they were unaware of what had just occurred. Lt. McCarthy made several Mayday calls from within the structure and activated his PASS device. Confusion erupted exteriorly on scene when trying to verify who was calling the Mayday, their exact location, and how they got into the basement.

The incident commander was aware that he had crews attempting to gain access into the basement from Side 2 but was unaware that there had been a floor collapse within the deli section of the structure. Simultaneously, FF Croom, a member of the fire fighter assistance and search team (FAST), was standing by outside  Lieutenant McCarthy’s point of entry when the Mayday calls came out. It is believed that FF Croom  knew where the Lt., was since he had gone in the structure with him earlier in the incident. FF Croom grabbed a tool, went on air, and rushed into the structure. The FAST and additional personnel on scene concentrated on Side 2 initially while other fire fighters followed an unmanned hoseline into the deli. Crews within the deli quickly discovered a floor collapse and reported hearing a PASS device alarming. Lt. McCarthy was immediately identified as missing during the first accountability check, but FF Croom was not accounted for as missing until the third accountability check, more than 50 minutes after Lt. McCarthy’s Mayday. After the fire was controlled, both victims were discovered side-by-side in the basement where the 1st floor had partially collapsed. They were found without their facepieces on and with SCBA bottles empty. the Lt’s. PASS device was still alarming. They were pronounced dead on scene. Four fire fighters and one lieutenant suffered minor injuries during the incident. No civilians were discovered within the structure.

Key contributing factors identified in this investigation include working above an uncontrolled, free-burning basement fire; interior condition reports not communicated to command; inadequate risk-versus-gain assessments; and, crew integrity not maintained.

NIOSH has concluded that, to minimize the risk of similar occurrences, fire departments should:

  • Ensure that all personnel are aware of the dangers of working above a fire, especially a basement fire, and develop, implement, and enforce a standard operating procedure (SOP) that addresses strategies and tactics for this type of fire.
  • Ensure that the incident commander (IC) receives interior status reports and performs/continues evaluating risk-versus-gain.
  • Ensure that crew integrity is maintained at all times on the fireground.
  • Ensure that the incident commander (IC) receives accurate personnel accountability reports (PAR) so that he can account for all personnel operating at an incident.
  • Ensure that a separate incident safety officer, independent from the incident commander, is appointed at each structure fire.
  • Ensure that fire fighters use their self-contained breathing apparatus (SCBA) and are trained in SCBA emergency procedures.

CONTRIBUTING FACTORS 

Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in the injury or fatality. NIOSH investigators identified the following items as key contributing factors in this incident that may have led to the fatalities:

  • Working above an uncontrolled, free-burning basement fire.
  • Interior condition reports not communicated to command.
  • Inadequate risk-versus-gain assessments.
  • Crew integrity not maintained.

Time Line from the Buffalo (NY) Fire Department Investigative Report

3:51 a.m. – fire crews were sent to 1815 Genesee Street in Buffalo. When they arrived, they were met by a resident who said he heard people trapped inside. Crews began searching the building, but were eventually ordered out as conditions deteriorated.

4:22 a.m. – Members of Rescue 1 entered the building to make sure all firefighters had evacuated the building. Less than two minutes later the floor in the rear of the building collapsed. Lt. McCarthy of Rescue 1 fell into the basement as the floor collapsed. according to the report, other members of Rescue 1 were unaware of the collapse and only reported hearing a loud noise. McCarthy began calling for help on his radio, but other members of Rescue 1 were unable to determine where the calls were coming from and left the building unaware that Lt. McCarthy was trapped.

4:23 a.m. – Firefighter Croom entered the building after hearing the calls for help. the report says he did not exit the building, apparently falling into the basement near Lt. McCarthy.

4:31 a.m. – An emergency head count was ordered to determine the identity of the missing firefighter. Lt. McCarthy was reported missing at that time, but FF Croom was not. Firefighters in the front of the store reported hearing a pass alarm, but could not reach it due to extreme fire conditions, a weakened floor and continuing collapse.

4:48 a.m. – all crews were ordered out of the building because it had become unsafe.

Later, concerns began to arise that FF Croom was missing. the report says he was erroneously reported in a remote area.

5:46 a.m. – On scene personal realize FF Croom is missing and likely inside the building.

6:10 a.m. – Another head count is taken and FF Croom is reported missing.

9:18 a.m. – the Recovery Group reports that the two missing firefighters had been located in the basement, covered in fallen debris.

9:32 a.m. – the debris is cleared and Recovery Group firefighters reach Lt. McCarthy and FF Croom.

Maintaining Situational Awareness

00-EOY-ss-buildingcollapseMaintaining focused situational awareness while recognizing and processing a wide latitude of incoming information and observations at complex and multiple alarm incidents is a significant challenge to even the most experienced of incident command teams. However, things can go wrong and they can go wrong in a rapidly escalating manner with little time to recover. A prominent double LODD incident from six years ago provides poignant lessons learned as does another history repeating event (HRE) from 1972.

The Ebenezer Baptist Church fire in Pittsburg, PA (2004) and the Hotel Vendome Fire in Boston, MA (1972) have a number of commonalities related to extended multi-alarm operations, building compromise and collapse and multiple line-of-duty deaths of operating fire service personnel. Although building type, construction features and systems are unique for each incident as are the circumstances that lead to the events, there are mission critical lessons to be reexamined or newly introduced if you’re not familiar with either event. This is especially true when we talk about operational challenges and adverse conditions that result in firefighter injuries and fatalities during overhaul and take-up phases of an incident.

Remember Situation Awareness, [SA], is the perception of environmental elements within a volume of time and space, the comprehension of their meaning, and the projection of their status in the near future. It is also a field of study concerned with perception of the environment critical to decision-makers in complex, dynamic situations and incidents.

Both the 2006 and 2007 Firefighter Near-Miss Reporting System Annual Reports identified a lack of situational awareness as the highest contributing factor to near misses reported. Situation Awareness (SA) involves being aware of what is happening around you at an incident to understand how information, events, and your own actions will impact operational goals and incident objectives, both now and in the near future. Lacking SA or having inadequate SA has been identified as one of the primary factors in accidents attributed to human error (Hartel, Smith, & Prince, 1991) (Nullmeyer, Stella, Montijo, & Harden, 2005). Situation Awareness becomes especially important in work related domains where the information flow can be quite high and poor decisions can lead to serious consequences.

To the Incident commander, Fire Officer or firefighter, knowing what’s going on around you, and understanding the consequences is mission critical to incident stabilization and mitigation and profoundly crucial in terms of personnel safety. The integration of Situational Awareness and Dynamic Risk Assessment is a mission critical element in strategic incident command management and company level tactical operations as we go forward into the next decade. We’ll expand on some posting in the near future and address Dynamic Risk Assessment in the context of building and occupancy profiling and operations. Additionally, maintaining a heightened sense of risk and safety integrity when operating within non-combat fire suppression modes or phases also requires due diligence, focused and fluid situational awareness coupled with concise monitoring of building conditions, indicators (both evident and projected) and taking conservative actions and postures to ensure personnel are not placed in high risk, no value positions that have a high potential for error likely outcomes.

Fire Fighter LODD after Being Trapped in a Roof Collapse During Overhaul of a Vacant/Abandoned Building. NIOSH recently published a report on a 2008 LODD that occurred in a vacant/ abandoned building. NIOSH Report F2008-0037. The full report is available HERE. Let’s look at some insights and overviews of that report.

Report Summary

On November 15, 2008, a 38-year-old male fire fighter  died after being crushed by a roof collapse in a vacant/abandoned building. Fire fighters initially used a defensive fire attack to extinguish much of the fire showing from the second-floor windows on arrival. After the initial knockdown, fire crews entered the second floor to perform overhaul operations. During overhaul, the roof collapsed with several fire fighters still inside, on the second floor. The victim and two other fire fighters were trapped under a section of the roof. Crews were able to rescue two fire fighters (who self-extricated), but could not immediately find the victim. After cutting through roofing materials, the victim was located by fire fighters, unconscious and unresponsive.

He was removed from the structure and transported to a local hospital where he was pronounced dead. Key contributing factors identified in this investigation include: dilapidated building conditions, incendiary fire originating in the unprotected structural roof members, inadequate risk-versus-gain analysis prior to committing to interior operations involving a vacant/abandoned structure, inadequate accountability system, lack of a safety officer, an inadequate maintenance program for self-contained breathing apparatus (SCBA) and a poorly maintained and likely inoperable personal alert safety systems (PASS), ineffective strategies for the prevention of and the remediation of vacant/abandoned structures and arson prevention.

Inherent Construction Issues

This incident occurred in a vacant unsecured residential structure which had experienced a previous fire approximately one year prior to this incident. During interviews with NIOSH investigators, fire fighters reported large amounts of fire showing from all windows on the second floor on arrival. Fire fighters also reported that the roof had burned through on the Side B/C and one fire fighter reported he could see the sky while ascending the interior stairs to perform overhaul. It is not known if the roof conditions were communicated to the incident commander before fire fighters were assigned to operate on the roof. The fire fighters were unaware of the conditions such as the exposed roof assembly, possible removal of rafter connectors (collar beams), and the use of a flammable liquid in the structural members of the roof and second floor attic area. The roof assembly (being unprotected) was directly involved as part of the fuel in this fire.

The large dormer on the A-side presents an identifiable inherent risk factor (due to the potential for structural compromise or failure) when found on 1.5 story bungalow style residential structure due to the integral manner in which the dormer structure, i.e., roof rafters, dormer framing and roofing boards along with the functionality of the ridge beam must function in order to retain structural integrity under fire conditions. The dormer may be actually supported at the upper end directly onto the roofing boards, which in turn are supported by the perpendicular roof rafters. This creates a potential area for pronounced degradation when exposed to direct or indirect flame impingement creating an area prone to early structural compromise and eventual failure.

Although the initial defensive strategy in fighting the fire was successful in knocking down the fire, the incident commander may have benefited from a continuous risk-versus-gain analysis before allowing crews to operate on interior during overhaul. The first arriving officer reported that he performed a walk around prior to allowing crews to enter the structure and the building appeared intact, but he would not have known of the alterations to the interior roof system and the removal of critical structural members. Interior condition and roof condition reports might have revealed the burned-through area of the roof, and tactics could have been altered to keep fire fighters off the roof and out of the structure.

Report Recommendations included;

  • Ensure that the incident commander conducts a risk-versus-gain analysis prior to committing to interior operations in vacant/abandoned structures and continues the assessment throughout the operations
  • Ensure SOPs are developed for fighting fires in vacant/abandoned buildings
  • Ensure that the incident commander maintains close accountability for all personnel operating on the fireground
  • Ensure that a separate incident safety officer, independent from the incident commander, is appointed at each structure fire
  • Ensure that a respiratory protection program is in place to provide for the selection, care, maintenance, and use of respiratory protection equipment, including PASS devices.

Additionally, municipalities and local authorities having jurisdiction should:

  • Develop strategies for the prevention of and the remediation of vacant/abandoned structures and for arson prevention.

Although there is no evidence that the following recommendations could have prevented this fatality, NIOSH investigators recommend that fire departments:

  • Ensure that an EMS unit is on scene and available for fire fighter emergency care at working structure fires
  • Develop inspection criteria to ensure that all protective ensembles meet the requirements of NFPA 1851, Standard on Selection, Care, and Maintenance of Protective Ensembles for Structural Fire Fighting and Proximity Fire Fighting
  • Be aware of programs that provide assistance in obtaining alternative funding, such as grant funding, to replace or purchase fire equipment that can support critical fire department operations.

Vacant or Unoccupied: Tactical Risk and Safety

I’ve commented on this subject a few times. We seem to do a lot of things at times out of common practice and repetition, you know; “We’ve always done it that way….” syndrome. There’s a resonating theme that is making its way around the fire service dealing with going to a defensive tactical posture at vacant or unoccupied structure fires.

This command posture leads to limiting interior operating engagement, while promoting a high degree of risk management. With that being said, there are also plenty of opinions on these types of policies as such, since this type of tactical effort may be contrary to the local “culture and traditions” of the responding agencies and may be a hard pill to swallow, since we’re in the job of “ fighting ALL fires..” Please refresh your memories on a past post on Tactical Entertainment HERE and HERE

Here are some basic definitions to keep us all on the same playing field;

Vacant; refers to a building that is not currently in use, but which could be used in the future. The term “vacant” could apply to a property that is for sale or rent, undergoing renovations, or empty of contents in the period between the departure of one tenant and the arrival of another tenant. A vacant building has inherent property value, even though it does not contain valuable contents or human occupants.

Unoccupied; generally refers to a building that is not occupied by any persons at the time an incident occurs. An unoccupied building could be used by a business that is temporarily closed (i.e. overnight or for a weekend). The term unoccupied could also apply to a building that is routinely or periodically occupied; however the occupants are not present at the time an incident occurs. A residential structure could be temporarily unoccupied because the residents are at work or on vacation. A building that is temporarily unoccupied has inherent property value as well as valuable contents.

Here’s a formulative question;

  • As a responding company, you arrive at the scene of a vacant or unoccupied structure. The building’s construction features and systems have inherent risk associated with the occupancy, (as is the case with nearly all of our structures and occupancies).
  • Your company determines that you’re going to go defensive, even though you probably could make a reasonably safe entry and engage in interior structural fire suppression.
  • Would there be any repercussions in your station, battalion/district/community or organization if you took this tactic?

Predictability of Occupancy Performance during Suppression Operations

2-2-2009 3-20-14 PMOur buildings have changed; the structural systems of support, the degree of compartmentation, the characteristics of materials and the magnitude of fire loading. The structural anatomy, predictability of building performance under fire conditions, structural integrity and the extreme fire behavior; accelerated growth rate and intensively levels typically encountered in buildings of modern construction during initial and sustained fire suppression have given new meaning to the term combat fire engagement.

The rules for combat structural fire suppression have changed, but we have yet to write the rule book from which the new games plans must be derived. We seek the elusive “Rosetta stone” that aligns and interprets the emerging and traditionalist acumen related to fire stream effectiveness, flow rates, cooling capacity, extreme fire behavior and fire dynamics, compartment fire theory, propagation and cooling capacity and tactical deployment all relate towards defining an engineering approach to firefighting tactics versus the manual, labor-driven tactics of line deployment and rudiment placement of water on a fuel source within the fire compartment (room).

It’s no longer just brute force and sheer physical determination that defines structural fire suppression operations. It begs to suggest that many of today’s incident commanders, company officers and firefighters lack the clarity of understanding and comprehension that correlate to the inherent characteristics of today’s buildings, construction and occupancies and the need for refined engine company operations within the modern building construction setting. We assume that the routiness or successes of our operations and incident responses equates with predictability and diminished risk to our firefighting personnel.

The work of such notable suppression theory pioneers as P. Grimwood, E. Hartin, S. Särdqvist and S. Svennson and the concepts surrounding 3D firefighting, B-SAHF and other emerging research from the NIST and UL are areas that today’s discerning and progressive fire officer and commanders must become well-informed and conversant. The quantitative scientific data and emerging concepts from continuing research and testing such as the NIST’s Wind Drive Fire Studies and UL’s The Structural Stability of Engineered Lumber in Fire Conditions are providing enlightenment on fire development, fuel controlled and ventilation controlled fire development, operational time-duration parameters and degradation and failure mechanisms related to compromise and structural collapse in occupancies.

Our current generation of buildings, construction and occupancies are not as predictable as past conventional construction, therefore risk assessment, strategies and tactics must change to address these new rules of combat structural fire engagement.

The fundamental compartment that comprised a typical room configuration in terms of area (square footage), volume (height/Width), furnishings (fire load package) and materials of construction (structural anatomy) found within conventional, legacy or heritage construction provided predictability in terms of fire suppression, fire behavior, operational time and survivability (civilian/firefighter). The dramatic changes since the early 1980’s in the evolution of modern building construction and the institutionalization of engineered structural systems (ESS) have created compartment (room) areas in excess 500 SF, volumes that are open and spaciously interconnected to other habitable space, fire load packages that create extreme fire behavior, compromising structural stability in shorter time spans creating decreasing interior operational time and requiring increasing fire flow rates and volume to sustain requisite extinguishment demands.

Commanders and Company Offices need to gain new insights and knowledge related to the modern building occupancy and to modify and adjust operating profiles in order to safe guard companies, personnel and team compositions. Strategies and tactics must be based on occupancy risk not occupancy type and must have the combined adequacy of sufficient staffing, fire flow and nozzle appliances orchestrated in a manner that identifies with the fire profiling, predictability of the occupancy profile and accounts for presumed fire behavior. Today’s engine company operations and fire suppression theory has to progress beyond the pragmatic approaches to fire suppression such as “Big Fire-Big Water principle.

When we look at various buildings and occupancies, past operational experiences; those that were successful, and those that were not, give us experiences that define and determine how we access, react and expect similar structures and occupancies to perform at a given alarm in the future. Naturalistic (or recognition-primed) decision-making forms much of this basis. We predicate certain expectations that fire will travel in a defined (predictable) manner that fire will hold within a room and compartment for a predictable given duration of time; that the fire load and related fire flows required will be appropriate for an expected size and severity of fire encountered within a given building, occupancy, structural system; in addition to having an appropriately trained and skilled staff to perform the requisite evolutions.

Executing tactical plans based upon faulted or inaccurate strategic insights and indicators has proven to be a common apparent cause in numerous case studies, after action reports and LODD reports. Our years of predictable fireground experience have ultimately embedded and clouded our ability to predict, assess, plan and implement incident action plans and ultimately deploy our companies-based upon the predictable performance expected of modern construction and especially those with engineered structural systems.

If you don’t fully understand how a building truly performs or reacts under fire conditions and the variables that can influence its stability and degradation, movement of fire and products of combustion and the resource requirements for fire suppression in terms of staffing, apparatus and required fire flows, then you will be functioning and operating in a reactionary manner, that is no longer acceptable within many of our modern building types, occupancies and structures. This places higher risk to your personnel and lessens the likelihood for effective, efficient and safe operations. You’re just not doing your job effectively and you’re at RISK. These risks can equate into insurmountable operational challenges and could lead to adverse incident outcomes. Someone could get hurt, someone could die, it’s that simple; it’s that obvious.

Considerations for changing fire flow rates, the sizing of hose line and the adequacies for fire flow demand and application rates, staffing needs for safe operations, considerations for defensive positioning and defensive operating postures must be considered, and it warrants repeating again; Reckless-Aggressive firefighting must be redefined in the built environment and associated with goal oriented tactical operations that are defined by risk assessed and analyzed tasks that are executed under battle plans that promote the best in safety practices and survivability within known hostile structural fire environments- with determined, effective and proactive firefighting

crystalBall1As the end of the year fast approaches and in turn the end of the decade, it amazes me how “fast” time seems to have passed. Certainly when looking back and reflecting upon the past year or the previous few years, each of us thinks and contemplates upon those events, milestones, anniversaries, highlights as well as those common everyday occurrences that seem to permeate back and forth in our minds and hang at times like the smoke from a smoldering contents fire. When reflecting, there are the good times as well as those that were not so good. There are those events that were life altering and changing that forever formulate a different view upon each of our respective worlds we live and work within. As well as those events that have provided us with the joys and virtue of what we do everyday as firefighters both on and off the job, at the firehouse and at home.

For each or us, the events that form and shape our worlds; our families at home and our families at the fire station and within the fire department or agencies we volunteer or work for, leave indelible marks upon us that at times formulate and transcend us. My good friend Chief Ben Waller reflected upon a number of issues and insights in his recent post that was right on the mark as did my partner Chief Doug Cline in his perspective of 2009 and for 2010. A lot has happened to this our Fire Service during the past ten years and most certainly in the past twelve months that has shaped and forged a new generation of firefighters and tempered the existing veterans. Stop and think about it.

Looking back at 2009 and in the waning decade, the one certainty that we all share is that we have the ability and look forward to a new year, a new decade and to new challenges. Prior to this week, the 2009 Firefighter LODD events that sadly have occurred seemed like it would pause and we’d end the year with no further events. Tragically, in the past few days, five additional line-of-duty deaths have been reported through the USFA. From the events of 9-11, to the seeds that were planted in Tampa and the crusade that was embarked upon to ensure everyone [has] the opportunity to go home, through the tragedy, wake-up call and the lessons-learned from Charleston. A lot has happened, many tears have been shed, alot was learned, with so much more work still remaining.

As of this posting, the United States Fire Service has borne ninety-three (93) LODDs this year. In comparison to previous years, this may finally indicate a turning point in the previous escalating trends in LODD we’ve experienced during the past decade. Take a moment to look through the USFA postings and the narratives of each of the firefighters who made the supreme sacrifice in 2009 and reflect upon the circumstances and events that lead to their respective LODD incident. Take the time to spend an evening reading through some of the recent or past reports published on the NIOSH Fire Fighter Fatality Investigation and Prevention Program web site. Look the History Repeating Events (HRE) and think about what you can do to champion changes in your organization, department or company to eliminate or reduce the likelihood for a similar event from occurring to you or your organization.

The formulative and diligent efforts of the NFFF and the Everyone Goes Home Program and the Sixteen Firefighter Life Safety Initiatives have made their mark in this decade and must continue to be embraced and institutionalized as we move forward to twenty ten. Don’t forget about the inroads made by the National Firefigher Near-Miss Reporting System and the knowledge being gained to reduce HRE. We must look at and examine the successes and the failures of our methodologies, processes, culture and perspectives and continue to seek behaviors and practices that make our job safer. When we focus our attention on Building Construction, Command Risk Management and Firefighter Safety and the essence of combat structural fires; Structural firefighting is what it’s all about, is it not? The fundamental nature and reason we have such veneration for firefighting and the fire service and all it entails, has a lot to do with going into burning buildings and fighting fire. But firefighting has its adverse consequences, with all too familiar costs, in the form of injuries, debilitating accidents and line of duty deaths. As a firefighter; to say that we love firefighting would be an understatement, BUT one issue that we need to address is the fact that there are many individual firefighters, companies and organizations that employ fireground operational practices that promote the “enjoyment and entertainment” of working a good job within the occupancy compartment of a structural fire in the building environment.

One of the formulative postings I published this past year focused on working that good job for the shear enjoyment of what and who we are; firefighters. It’s worth repeating again, since this is an opportune time to reflect. Today’s incident scene and structural fires are unlike those in past decades and will continue to challenge us operationally when confronted with structural fire engagement and combat operations. Operationally, we need to be doing the right thing, for the right reason in the right place to increase our safety and incident survivability.

We also can share the belief and understanding that we at times may have found ourselves staying too long in the wrong place, operating tactically in an adverse environment with known hazards that do not have value, for nothing other than the enjoyment of nozzle and operating time in the fire. We have a tendency when working a room and contents, compartment fire or a structural fire in the building environment placing operating companies and personnel in high hazard environments- sometimes at the expense of justifying our own entertainment value in working the job, the assignment or in maintaining the interior operational interface. Think about it.

We need to stop “entertaining” ourselves. Don’t mistake determined, effective and proactive firefighting with that of reckless, baseless and risk-preferring and self-indulging firefighting. There is a difference. The job is dangerous, it has risks, we are not invincible, and we can die; at any alarm, in any fire, at anytime for any number of reasons. But it’s tragic when we die for all the wrong reasons. Think about the definitions; think about how they apply to you, your personnel, your company or your operations; past, present or future. More importantly, think about when and where you’ve found yourself doing any one of these; could the outcome have been different?

The demands and requirements of modern firefighting will continue to require the placement of personnel within situations and buildings that carry risk, uncertainty and inherent danger. As a result, risk management must become fluid and integrate all personnel. We must manage dynamic risks with a balanced approach of effective assessment, analysis and probability within command decision making that results in safety conscious strategies and tactics.

On any given day, at any give alarm, the dynamics around us at times may be in or out of our direct control. We may not be able to see what the cards have in store for us, BUT we must ensure we use every fragment of training, fortitude, knowledge, skills, courage, bravery, insights, luck and sometimes (other divine) intervention to get us through. We must have the fortitude and courage to be both safety conscious and measured in the performance of our sworn duties while maintaining the appropriate balance of risk and bravery.
• The demands and requirements of modern firefighting will continue to require the placement of personnel within situations and buildings that carry risk, uncertainty and inherent danger.

• As a result, risk management must become fluid and integrate all personnel.

Measured: Calculated; deliberate, careful; restrained, think, considered, confident, alternatives, reasoned actions, in control, self assured, calm…

There is a melting of both pragmatic aggressive firefighting with measured and deliberate tactical approaches. It’s a balance and equilibrium; the question is do you know when to recognize that balance, where it exists and how not to cross that adverse threshold?

Our current generation of buildings, construction and occupancies are not as predictable as past Conventional Construction; Risk assessment, strategies and tactics must change to address these new rules of structural fire engagement. You need to gain the knowledge and insights and to change and adjust your operating profile in order to safe guard your companies, personnel and team compositions.

Looking Forward through the Rear View Mirror; remember the past, recall those history repeating events that seem to manifest themselves time and time again; are we ever going to learn. I truly believe we are starting to finally “get it”-even if it’s on a smaller incremental scale, it’s a starting point. Remember the lessons from those events that have impacted you, your department, your community and the fire service; from close-calls to near-miss events; from minor or debilitating injuries to the tragedy and sorrow of a LODD event.

As we transition into a new year, and as plans begin to take place that frame and outline the year’s activities, foremost in this planning, preparation, scheduling and outlook should be those activities and commitments that training, education and skill development can be implemented and enhanced. Take the initiative to recognize and identify training and operational gaps and distinguish the risk and options available to lessen or eliminate the risk and reduce the gap deficiencies. Take the time to implement effective, accurate and frequent training and skill development drills, training curriculums and programs. Don’t sacrifice or forego on this mission critical area when so much is at stake in the domain of combat structural fire suppression. Understand the predictability of performance in the buildings and occupancies not only in your jurisdiction, first or second-due areas, but also in those areas that you may be called upon to respond to for greater alarms or mutual aid. Understand the structural anatomy of your community. Remember Building Knowledge = Firefighter Safety. Understand the fomulative issues affecting engineered structural systems (ESS) and the change in operational deployment and tactics on the fire ground. Keep an eye in the rear view mirror; learning from the wisdom and knowledge from where you’ve been, what you’ve done and all your past experiences and practice; but at the same time focusing on the road before you with keen attentiveness on situational awareness, anticipating error-likely conditions and balanced risk assessment and operational management in both your strategic and tactical deployments.

We don’t know what’s in the cards on any given day, but the citizens we protect can rest assured, we will do our jobs as firefighters, to the best of our abilities, because of who we are; today, in 2010 and certainly well into the next decade and beyond. 

Ensure you’re glancing occasionally in your rear view mirror to monitor where you’ve been, while driving your initiatives, programs, processes and actions forward. Above all, maintain the courage to be safe.

Remembering Brackenridge 1991 Floor Collapse and LODD

12-21-2009 9-53-23 PMRemembering Brackenridge, Pennsylvania December 20, 1991: Four Firefighters Killed, Trapped by Floor Collapse

Four volunteer firefighters died when they were trapped by a partial floor collapse during a structure fire in Brackenridge, Pennsylvania, on the morning of December 20, 1991. All four were members of a mutual aid truck company that had responded to the early morning incident and were assigned to prevent fire extension from the basement to the ground floor of a 2-story building. Although they were wearing full protective clothing and using self-contained breathing apparatus, it appears that they were overwhelmed by the severe fire conditions that erupted when a section of the ground floor collapsed into the basement. The collapse cut off their primary escape path, and the fire burned through their hose line, leaving them without protection from the flames.  

 SUMMARY OF KEY ISSUES

  • Situation: Fire in enclosed room in basement. Unable to locate fire because of smoke. Smoke and heat increasing, but no visible fire.
  • Structure: Appeared to be heavy concrete construction. Actually thin concrete floors supported by unprotected steel.
  • Contents: Furniture refinishing business. Quantities of flammable finishes and solvents in basement.
  • Exits: One entrance/ exit on each level; no alternate exits.
  • Structural Collapse: Floor section collapsed between interior crew and their only exit. Fire overwhelmed crew.
  • Rescue Attempts: Valiant rescue efforts proved unsuccessful. Unsure if missing members fell into basement or were trapped on ground floor.
  • Incident Command: No formal command system or personnel accountability in place. Chief of first-due company in command of incident; Assistant Chiefs assigned to basement and ground floor.
  • Information: No pre-fire plan and no detailed knowledge of occupancy. Clues of structural danger not recognized as fire conditions increased
  • Communications: Radio system inadequate for current needs.
  • Response: Independent volunteer companies. Mutual aid requested on arrival and additional companies called in succession.
  • Weather: Extremely cold night, predawn hours. Problems with frozen hydrants.
  • Water System: Weak supply. Extensive mutual aid and long relays needed to protect exposures.

The analysis of this incident provides several valuable lessons for the fire service. Unfortunately these are all revisited lessons, not new discoveries. These firefighters died in the line of duty, while conducting operations that appeared to be routine, and were unaware of the situation that was developing below them. They died in spite of the fact that they were experienced, they were operating with a standard approach to operational safety, and they were the object of repeated rescue attempts by highly capable comrades.

There are several factors that could have provided warning or changed the outcome of this situation. Like most accidents, this situation was the result of a number of problems that came together under the worst possible circumstances. Firefighting obviously involves inherent dangers that must be accepted by its practitioners. The important messages for the fire service are to identify risk factors in advance of an incident and to develop mechanisms to react appropriately when critical situations present themselves.

This situation bears distinct similarities to other incidents that have claimed the lives of several firefighters in the past. The lessons that must be derived from this incident are not a condemnation of the actions or judgment of anyone who was involved in the situation; they simply identify information that can help to prevent this type of accident from occurring in the future.

Truss and Engineered Systems Placards

11-22-2008 9-05-24 PMThe Aldridge-Benge Firefighter Safety Act of 2008 became law on December 13, 2009 after unanimously passing the Florida House and Senate in 2008. The new law is named in honor of two Orange County, Florida  Firefighters, Todd Aldridge and Mark Benge, who died in 1989 after the roof of a gift shop collapsed; the bill is called the Aldridge-Benge Firefighter Safety Act. For a copy of the Act, HERE

 The Aldridge-Benge Firefighter Safety Act will require owners of any commercial, industrial, or any multi-unit residential structure, to mark these buildings in a manner that identifies them as light-frame truss-type construction. A sign or symbol will alert firefighters of the construction material and allow them to modify their tactics for fighting fires in buildings.

633.027 Buildings with light-frame truss-type construction; notice requirements; enforcement.

(1) The owner of any commercial or industrial structure, or any multiunit residential structure of three units or more, that uses light-frame truss-type construction shall mark the structure with a sign or symbol approved by the State Fire Marshal in a manner sufficient to warn persons conducting fire control and other emergency operations of the existence of light-frame truss-type construction in the structure.

(2) The State Fire Marshal shall adopt rules necessary to implement the provisions of this section, including, but not limited to:
(a) The dimensions and color of such sign or symbol.
(b) The time within which commercial, industrial, and multiunit residential structures that use light-frame truss-type construction shall be marked as required by this section.
(c) The location on each commercial, industrial, and multiunit residential structure that uses light-frame truss-type construction where such sign or symbol must be posted.

(3) The State Fire Marshal, and local fire officials in accordance with s. 633.121, shall enforce the provisions of this section. Any owner who fails to comply with the requirements of this section is subject to penalties as provided in s. 633.161

What we don’t have is a unified national standard, nor do we have these systems in all states. The political strife and lobbying backed by special interest groups and mfg. associations that DO NOT Support these types of placard systems is appalling and inexcusable. Truss roof signs are required by the New Jersey State Uniform Fire Code for buildings, which utilize either a floor or roof assembly consisting of truss construction. A truss sign gives early warning to fire and emergency service members that the roof and/or floor may be subject to early collapse in the event of a fire condition.

This sample language is based on recommendations in the National Institute for Occupational Safety and Health (NIOSH) report entitled “NIOSH Alert: Preventing Injuries and Deaths of Firefighters due to Truss System Failures.”
The report states: “Consider placing building construction information outside the building. Include
information about roof and floor type.

The NIOSH report also recommends as part of pre-fire planning to: Record data regarding roof and floor construction (e.g., wooden joist, wood truss, steel joist, steel truss, beam and girder, etc.) [NFPA 2003]. The sample language below provides building labeling that identifies the building’s construction type, is simple yet logical, and should allow firefighters to quickly know the building’s floor and roof construction materials, promoting better and more complete information on the fireground and increased firefighter safety.

xxx Identification of structural construction. Structural construction types shall be identified by a sign or signs, in accordance with the provisions of this section.

xxx.1 Signs. Signs shall be affixed where a building or a portion thereof is classified as Group A, B, E, F, H, I, M, R-1, R-2, R-4 or S occupancy. The owner of the building shall be responsible for the installation of the sign.
xxx.2 New buildings and buildings being added to. Signs shall be provided in newly constructed buildings and in existing buildings where an addition that extends or increases the floor area of the building. Signs shall be affixed prior to the issuance of a certificate of occupancy or a certificate of compliance.

xxx.3 Existing buildings. Signs shall be provided in existing buildings. Signs shall be affixed within ninety days of being notified in writing by the Code Enforcement Official.

xxx.4 Contents of signs. Signs shall consist of a diagram 6 inches (152.4 mm)in height and width, with a stroke width of ¼ inch (6.4 mm). The sign background shall be reflective white in color. The diagram and contents shall be reflective red in color, conforming to Pantone matching system (PMS) #187. Where a sign is directly applied to a door or sidelight, it may be a permanent non-fading sticker or decal. Signs not directly applied to doors or sidelights shall be of sturdy, non-fading, weather resistant material.

xxx.5 Identification of construction classification. Signs shall contain the roman alphanumeric designation of the construction classification of the building, in accordance with the provisions for the classification of types of construction (types I through V) of the building code. The roman numeral designating construction classification shall be 1 inch (25.4 mm) minimum in height and have a stroke width of ¼ inch (6.4 mm) minimum, and it shall be reflective white in color on a background of reflective red.

xxx.6 Identification of year of construction. Signs shall indicate the building’s year of construction or major reconstruction. The arabic numeral indicating year of construction shall be 1 inch (25.4 mm) minimum in height and have a stroke width of ¼ inch (6.4 mm) minimum, and it shall be reflective white in color on a background of reflective red.

xxx.7 Identification of structural construction types. Signs shall contain the alphabetic designations identifying the structural construction types used in the building, as follows:

“W” shall mean sawn joist/rafter construction, wood members
“I” shall mean engineered I-joist construction, wood members
“S” shall mean steel construction
“T” shall mean truss type construction
“C” shall mean concrete construction

The Aldridge-Benge Firefighter Safety Act. The law was named in honor of Orange County firefighters Todd Aldridge and Mark Benge, who died in 1989 after the truss roof of a gift shop collapsed. Under the new law, owners of any commercial, industrial or multi unit residential structure, have to clearly mark if their buildings have lightweight roof or floor trusses, allowing firefighters to change their tactics when working in these types of structures

633.027 Buildings with light-frame truss-type construction; notice requirements; enforcement
(1) The owner of any commercial or industrial structure, or any multiunit residential structure of three units or more, that uses light-frame truss-type construction shall mark the structure with a sign or symbol approved by the State Fire Marshal in a manner sufficient to warn persons conducting fire control and other emergency operations of the existence of light-frame truss-type construction in the structure.
(2) The State Fire Marshal shall adopt rules necessary to implement the provisions of this section, including, but not limited to:
(a) The dimensions and color of such sign or symbol.
(b) The time within which commercial, industrial, and multiunit residential structures that use light-frame truss-type construction shall be marked as required by this section.
(c) The location on each commercial, industrial, and multiunit residential structure that uses light-frame truss-type construction where such sign or symbol must be posted.
(3) The State Fire Marshal, and local fire officials in accordance with s. 633.121, shall enforce the provisions of this section. Any owner who fails to comply with the requirements of this section is subject to penalties as provided in s. 633.161.

The use of trusses in building construction presents a great danger to firefighting personnel when those structures are involved in fire conditions. Because of the inherent danger firefighters must face while operating within these buildings, a Truss Identification Program (TIP) has been instituted to alert personnel of the danger prior to beginning fire suppression operations. The Truss Identification Program is intended to alert the members of the Chesapeake Fire Department with pertinent pre-plan information before firefighting forces are committed to an interior attack.

The TIP shall be an ongoing program applied to all commercial buildings inspected by the Chesapeake Fire Department.

City of Greencastle, Indiana

The City of Greencastle, Indiana and the Greencastle Fire Department recently enacted and approved an Engineered Lumber ID Program consisting of a sticker that is used for quick recognization of potential Collapse Dangers associated with Engineered Lumber constructed buildings. The sticker is placed on every electrical meter of all residential & commercial buildings with Engineered Lumber construction built after May 13th 2008. The news release states that; the use of this type of lumber in building construction presents a great danger to firefighting personnel when those structures are involved in fire conditions. By design, the Engineered Lumber in floor and roof assemblies will collapse, without warning, after being exposed to heat or flame contact for a very short period of time. Because of the inherent danger firefighters must face while operating within these buildings, an Engineered Lumber Identification Program (ELIP) has been instituted to alert personnel of the danger prior to beginning fire suppression operations.

The Engineered Lumber Identification Program is intended to alert the members of the Greencastle Fire Department with pertinent pre-plan information before firefighting forces are committed to an interior attack. The sticker is unobtrusive and is placed directly on a meter box, for example, and alerts the FD if either the floor joists and/or the trusses are made of and Engineered Lumber System and materials. The fire officers are already checking the utility boxes on all fires as part of their initial size-up. The ELIP shall be an ongoing program applied to all residential & commercial buildings inspected by the Greencastle Fire Department.

ORDINANCE 2008 – 4 states; AN ORDINANCE REQUIRING A REFLECTIVE SYMBOL ON STRUCTURES USING ENGINEERED LUMBER
WHEREAS, many new building structures currently use engineered lumber in their construction;
WHEREAS, some types of engineered lumber burn at a rate faster that other types of lumber; and
WHEREAS, in fighting fires, it would be helpful to know the types of materials used in the construction of a structure.

NOW THEREFORE be it ordained by the Common Council of the City of Greencastle as follows:
1. Definitions:
a. Engineered Lumber shall mean prefabricated I-joists, truss joists, and truss rafters, and laminated beams and studs.
b. Structure shall mean primary, secondary and accessory structures as defined in the Greencastle Zoning Code that have electrical meters that serve the structure.

2. All structures constructed with engineered lumber after the effective date of this ordinance must have a reflective symbol affixed to each electrical meter serving the structure.

3. The reflective symbol shall be in the form of a sticker, issued by the City of Greencastle that states that the structure is constructed with engineered lumber

4. Any person violating this ordinance by refusing to use the reflective symbol or by removing the reflective symbol shall be subject to a fine in an amount of $25.00 per violation. Each day that a violation occurs shall constitute a separate violation, subject to a separate fine.

5. The owner of any structure that was constructed with engineered lumber prior to the effective date of this ordinance is requested to place the reflective symbol on the electrical meter serving the structure on a voluntary basis.

This is another great example how local level insights, actions and legislation can go a long way in supporting fire service operational challanges as they relate to building construction systems, methodologies and materials. Remember, We can certainly work diligently AND cooperativley with local government officials to enhance incident operations and make our jobs safety, one step at a time….

A million dollar Baltimore County, Maryland  home was destroyed Sunday December 13, 2009  by a fire that tore through the 4,700-square-foot structure with such intensity that firefighters were forced to battle the flames from the exterior. Shortly after 21:00 hours, Baltimore County Fire Dispatch alerted crews for Fire Box 50-2 at 12607 Nancy Lee Court in the Worthington Trace subdivision in the Chestnut Ridge area. As firefighters were responding, dispatch advised they were receiving multiple calls to 911, with some reporting the entire house was on fire. While en route to the scene, Chestnut Ridge Volunteer Fire Company Captain Dan Uddeme reported heavy fire was visible and requested a 2nd alarm and a Tanker Strike Team as the house sits in an area without fire hydrants. Upon arrival, Capt. Uddeme reported fire had consumed the entire 2nd floor and roof area and was spreading. Firefighters were forced to use exterior operations due to the heavy volume of fire. Responding units set up for rural water operations, shuttling more than 17,000 gallons of water from an underground tank on Greenspring Avenue and Walnut Avenue near the scene. Reisterstown Volunteer Fire Companys Engine 412 was also utilized for its Compressed Air Foam System, with several handlines and the ladder pipe from Glyndon Volunteers Truck 404 flowing foam. While watching the video, take the time to listen to the wind howling across the mic and observe the intesity level of the fire severity and propogation in the Charlie side. This provides an opportunity for those that are not familiar with the NIST Wind Driven Fire Studies or the PWC (VA) Kyle Wilson LODD to take some time to read about the affects of wind on incident operations, strategies and tactical personnel safety. This was a 4,700 SF large volume residential structure. Think about the performance and your deparment’s capabilities? Remember, it’s not “just” a house fire

Take a look at the Prince William County (VA) Fire & Rescue case study information related to Technician I Kyle Wilson – LODD Report. This event: Technician Kyle Wilson died in the line of duty on April 16, 2007 while performing search and rescue operations at a house fire on Marsh Overlook Drive, located in the Woodbridge area of Prince William County. On that day, Technician Wilson was part of the firefighter staffing on Tower 512 which responded to the house fire that was dispatched at 0603 hours. The Prince William County area was under a high wind advisory as a nor’eastern storm moved through the area. Sustained winds of 25 mph with gusts up to 48 mph were prevalent in the area at the time of the fire dispatch to Marsh Overlook Drive. Initial arriving units reported heavy fire on the exterior of two sides of the single family house and crews suspected that the occupants were still inside the house sleeping because of the early morning hour. A search of the upstairs bedroom commenced for the possible victims. A rapid and catastrophic change of fire and smoke conditions occurred in the interior of the house within minutes of Tower 512’s crew entering the structure. Technician Wilson became trapped and was unable to locate an immediate exit out of the hostile environment. Mayday radio transmissions were made by crews and by Technician Kyle Wilson of the life-threatening situation. Valiant and repeated rescue attempts to locate and remove Technician Wilson were made by the firefighting crews during extreme fire, heat and smoke conditions. Firefighters were forced from the structure as the house began to collapse on them and intense fire, heat and smoke conditions developed. Technician Wilson succumbed to the fire and the cause of death was reported by the medical examiner to be thermal and inhalation injuries.

National Institute of Standards and Technology – NIST Wind Driven Fire Research HERE Smoke and heat spreading through the corridors and the stairs of a building during a fire can limit building occupants’ ability to escape and can limit fire fighters’ ability to rescue them.  Changes in the building’s ventilation or presence of an external wind can increase the energy release of the fire.  This can also increase the spread of fire gases through the building.  In some cases, such as the Cook County Administration Building fire in October 2003, the fire gas flow, into the corridors and the stairway prevented fire fighters from suppressing the fire from inside the structure.  This fire resulted in 6 building occupant fatalities and fire fighter injuries in the stairway.  The Fire Department of New York City has experienced many wind driven fire incidents which have resulted in fire fighter fatalities and injuries.

What tactics or tools are appropriate for use with a wind driven fire and how should the tactics or tools be implemented?  Positive Pressure Ventilation (PPV) is being used by fire departments on smaller structures, such as single family homes, to control the fire flow by introducing pressure from the front door and venting the house through a strategic exit opening.  If done correctly, this tactic can remove significant amounts of heat and smoke from the structure, thus improving the fire fighters’ working environment and improving the chances of survival for the building occupants.  NIST has completed several studies which have a two fold impact: 1) providing guidance on the safe use of PPV and 2) characterizing and validating the modeling of PPV with a computational fluid dynamics (CFD) computer model, so that the model can be used as a training tool for the fire service.     Floor TrussOf the many issues affecting the Fire Service, the prevailing challenge that has a pronounced impact on operational safety is the assimilation of engineered structural systems (ESS) into mainstream building design and construction. The presence of engineered structural systems (ESS) are no longer considered to be an innocuous feature in a given building or occupancy; it is the predominate feature in nearly all current construction, renovation and adaptive reuse or infill applications. It has become far more than just concerning ourselves with the presence of a simple light-weight or “engineered” truss roof system or a wood I-beam  floor assembly.

    There is a new lexicon of building construction components and systems that must be added to your operational safety vocabulary and incident action plans. There is a new terminology, applications and a knowledge base to learn that will support operational excellence and support the integrity of incident safety performance of companies and personnel. Do you know what they represent and how these components, assemblies and systems may affect or influence an incident?

    The fire service continues to apply the term “light weight construction” to a wide variety of building construction and systems. This expression has become a miss-application of both term and the correlation of risk and severity related to operational profiling. In other words, we apply and express the use of “light weight construction” for all types of engineered components, systems, designs and assemblies in nearly all types of building construction and occupancy use. Although the roots of the term can be traced back to the early 1980′s, and its application to the (then) emerging use of trussed roofing systems and the advent of wood I-beam floor supports (sans solid dimensional lumber joists), the use of the terminology in today’s context of risk assessment, strategic and tactical management and deployment models and within the context of incident operational tactics is no longer applicable, valid or suitable. It must be expanded into a more specific and descriptive level of classification and correlation.

    For the most part, when discussing buildings and occupancies, aside from classifications related to code type or class as an element of fire resistance; the emphasis has been to differentiate between conventional and engineered construction, and the application of the term “light weight construction”. I continue advocating and promoting through my lectures that it’s much more than this when looking at the spectrum of construction and the structural anatomy of buildings. Current and past generations of buildings, construction and occupancies can be more accurately differentiated and classified within six (6) expanding categories in the following Building Construction Systems;

    ·        Heritage:              Pre-1900
    ·        Legacy:                1900-1949
    ·        Conventional:      1950-1979
    ·        Engineered:         1980-2009     Current into 2010…
    ·        Blended Hybrid:  1995-2009     Current into 2010…
    ·        Enigmatic:            2010-             Projected
     
    We’ll discuss these six classifications in greater details in future postings here and expand the level of details on the CommandSafety.com and Buildingsonfire.com sites. Our current generation of buildings, construction and occupancies are not as predictable as past “conventional” construction, therefore risk assessment, strategies and tactics must change to address the advancement of new rules of combat structural fire engagement. But if you don’t understand or know what and how those changes in predictability have occurred, you may be operating with a false sense of operational risk and safety margin.

    3183630397_6104ecd8cd_bWhen we focus our attention on Building Construction, Command Risk Management and Firefighter Safety and the essence of combat structural fires; Structural firefighting is what it’s all about, is it not? The fundamental nature and reason we have such veneration for firefighting and the fire service and all it entails, has a lot to do with going into burning buildings and fighting fire.

    We enjoy it tremendously; we have fun at, because of who we are and what we do-as firefighters. It’s the job and it’s a calling. Firefighting; It’s not something you do, it’s something you are. But firefighting has its adverse consequences, with all too familiar costs, in the form of injuries, debilitating accidents and line of duty deaths.

    As a firefighter; to say that we love firefighting would be an understatement, BUT one issue that we need to address is the fact that there are many individual firefighters, companies and organizations that employ fireground operational practices that promote the “enjoyment and entertainment” of working a good job within the occupancy compartment of a structural fire in the building environment.

    Today’s incident scene and structural fires are unlike those in past decades and will continue to challenge us operationally when confronted with structural fire engagement and combat operations. Operationally, we need to be doing the right thing, for the right reason in the right place to increase our safety and incident survivability. We also can share the belief and understanding that we at times may have found ourselves staying too long in the wrong place, operating tactically in an adverse environment with known hazards that do not have value, for nothing other than the enjoyment of nozzle and operating time in the fire. We have a tendency when working a room and contents, compartment fire or a structural fire in the building environment placing operating companies and personnel in high hazard environments- sometimes at the expense of justifying our own entertainment value in working the job, the assignment or in maintaining the interior operational interface. Think about it.

    We need to stop “entertaining” ourselves. Don’t mistake determined, effective and proactive firefighting with that of reckless, baseless and risk-preferring and self-indulging firefighting. There is a difference. The job is dangerous, it has risks, we are not invincible, and we can die; at any alarm, in any fire, at anytime for any number of reasons…..Let me leave you with some new thoughts and concepts related to operational safety and the definitions that I’ve come to develop that may support apparent or contributing causes to many of the fire service’s undesired events or incidents. Think about the definitions; think about how they apply to you, your personel, your company or your operations; past, present or future. More importantly, think about when and where you’ve found yourself doing any one of these….could the outcome have been different?

    My most recent post that stated and advocated that fire suppression tactics must be adjusted for the rapidly changing methods and materials impacting all forms of building construction, occupancies and structures. The need to redefine the art and science of firefighting is nearly upon us. HERE

    A common theme has been evident here at FRI in Dallas related towards the continued identified need for the fire service to begin what I call a Tactical Renaissance. I discussed what defines and identifies the attributes and suggested what needs to be instituted earlier in the week at FRI.

    One thing is not only clearly self-evident throughout common themes this past week, but also resonates through direct discussions with various Executive Chief Officers, Field Operations Commanding Officers and Training Officers. They all agree that the fire service needs to do something-and soon in redefining our strategies, tactics and way we are doing business in the streets. “The times they are a changin….”

    The Job: Doing the Right Thing, At the Right Time, for the Right Reasons

    On Monday August 24th, two Buffalo (NY) Firefighters sustained injuries and died in the line-of-duty (LODD) during the conduct of operations at a two-story Type III (brick and joist) commercial occupancy in Buffalo, New York. The 911 call reported someone may be trapped inside the 1815 Genesee Street occupancy just before 4:00 hours. (more HERE and HERE)
    Arriving companies reported a working fire in the basement. Lt. Charles “Chip” McCarthy of Rescue Company 1 was conducting search and rescue operations, when the number one floor collapsed, plunging him into the basement. Firefighter Jonathan Croom of Ladder Company 7 entered the building to search for Lt. McCarthy in response to the Mayday call. The mayday was called a little more than a half-hour into the incident. At about 4:09 AM, Command gave the following report:
    “Twenty minutes into 1815 Genesee Street. Two-story ordinary. We are now operating three, inch and three quarter lines. We are doing an aggressive interior search. We still have a report of civilians trapped. We believe they may be in the basement. We are going to hold everybody we have including the two and one.”
    Command soon asks for the balance of the second alarm. According to published reports and radio communication transcripts, companies reported difficulty getting into a basement door and deteriorating conditions on the number one floor. Genesee Street Command reported at the thirty-minute mark:
    “Thirty minutes into 1815 Genesee Street. Still continuing to operate all hands. We will continue to hold the second alarm.” ( MP3 Communications, HERE)
    There’s been a lot of talk of recent regarding the fire service cultural divides. We should take a moment to pause and reflect on the job of firefighting in light of the Buffalo LODD’s and refocus and recognize the job of firefighting has inherent risks, and the job requires us to; Do the Right Thing, at the Right Time, for the Right Reasons.
    Lt. Chip McCarthy and Firefighter Jonathan Croom were doing the right thing, when deployed on the primary search and rescue assignment on the first-due, and the subsequent search and rescue on the RIT/mayday assignment. Their sacrifice in the line-of-duty, reflects the honor, courage, protection, fortitude and duty of the fire service.
    Operations at buildings of Ordinary and Heavy Timber construction require a clear understanding of their inherent structural features and conditions, and a firm knowledge of structural degradation and compromise resulting from fire suppression operations. Firefighter safety and operational integrity is contingent upon knowing how these buildings perform and may be impacted by structure fires within their occupancies.     I had the privilege to spend some time recently with a tremendous group of knowledgeable and dedicated command and company officers at the 2009 Arizona Fire Service Leadership Conference hosted by the Arizona Fire Chief’s Association in Glendale, Arizona. Chief Ron Dennis, the Executive Director of the AFCA did a wonderful job of planning and facilitating a rich and rewarding program of presentations by some of the fire service’s foremost leaders that included Chief Charlie Dickinson, Deputy Administrator, USFA, (ret), Chief Dennis Rubin, Chief Jeff Johnson, Chief Alan Brunacini (ret), Chief Rich Marinucci, Chief Greg Cade (ret), Howard Cross, Chief Kevin Brame and Chief Bill Jenaway to name a few.

    I had the opportunity to present a though provoking program addressing current trends in Building Construction, and the impact and influence on Command Risk Management and Firefighter Safety. An interesting discussion prevailed during one segment of the presentation that I’d like to share with you. While on shift today or at the station this evening or during the week, think about the following and where and how you fit into the big scheme of things. Explore and discuss the ramifications of risk-preferring and self-indulging firefighting. There’s more here than meet’s the eye, IF you look hard enough.

    Risk-Preferring and Self-indulging Firefighting
    Don’t mistake determined, effective and proactive firefighting with that of reckless, baseless and risk-preferring and self-indulging firefighting. There is a difference, a big difference! When we address relationships of Building Construction, Command Risk Management and FireFighter Safety with the occupancy and structural environment, all personnel, regardless of rank, need to equate the occupancy risk with strategic and tactical incident action plans.These safely compliment the identified firefighting operation risk, with the projected building risk profile and interface appropriate behavioral characteristics in the task level firefighting activities. Again, equating building, occupancy risk profiles with determined, effective and proactive firefighting.

    The traditional attitudes and beliefs of equating aggressive firefighting operations in all occupancy types coupled with the correlating, established and pragmatic operational strategies and tactics MUST not only be questioned, they need to be adjusted and modified; risk assessment, risk-benefit analysis, safety and survivability profiling, operational value and firefighter injury and LODD reduction must be further institutionalized to become a recognized part of modern firefighting operations.

    Risk-Preferring and Self-Indulging Firefighting It’s no longer just brute force and sheer physical determination that define structural fire suppression operations. Aggressive firefighting must be redefined and aligned to the built environment and associated with goal oriented tactical operations that are defined by risk assessed and analyzed tasks that are executed under battle plans that promote the best in safety practices and survivability within know hostile structural fire environments. Consider the following definitions as they relate to defining structural combat fire suppression operations.

    Aggressive and Measured Approach
    Aggressive: Assertive, bold, and energetic, forceful, determined, confident, marked by driving forceful energy or initiative, marked by combative readiness, assured, direct, dominate…

    Measured: Calculated; deliberate, careful; restrained, think, considered, confident, alternatives, reasoned actions, in control, self assured, calm…

    You be the judge as to what should be appropriately defining interior fire suppression operations.

    It’s all about understanding the building-occupancy relationships and integrating; construction, occupancies, fire dynamics and fire behavior, risk, analysis, the art and science of firefighting, safety conscious work environment concepts and effective and well-informed incident command management. This is what it’s going to take to truly provide a means for “everyone to go home”. It’s Occupancy Risk not Occupancy Type. Many of today’s incident commanders, company officers and firefighters lack the clarity of understanding and comprehension that correlate to the inherent characteristics of today’s buildings, construction and occupancies. We assume that the routiness of our operations and incident responses equates with predictability and diminished risk to our firefighting personnel.

    Rules of Engagement for Structural Firefighting

    The International Association of Fire Chiefs (IAFC) is committed to reducing firefighter fatalities and injuries. As part of that effort, the IAFC Safety, Health and Survival (SHS) Section has developed DRAFT “Rules of Engagement for Structural Firefighting” to provide guidance to individual firefighters and incident commanders regarding risk and safety issues when operating on the fireground.

    The intent is to provide a set of model procedures to be made available by the IAFC to fire departments as a guide for their own standard operating procedures development.

    The direction provided to the project team by the Section leadership was to develop rules of engagement with the following conceptual points:

    • Rules should be a short, specific set of bullets
    • Rules should be easily taught and remembered
    • Rules should define critical risk issues
    • Rules should define “go” ‐ “no‐go situations
    • A champion lesson plan should be provided

    Early in development the rules of engagement, it was recognized that two separate rules were needed –one set for the firefighter, and another set for the incident commander. Thus, the two sets of rules of engagement described in this document.

    Each set has several commonly stated bullets, but the explanations are described somewhat differently based on the level of responsibility (i.e., firefighter vs. incident commanders).

    The draft documents are currently open for public comment until the FRI conference in Dallas (August 25‐29, 2009).

    Fire/EMS Safety, Health and Survival Week: Day Seven; “The Courage to be Safe” Protect Yourself: Your Safety, Health and Survival Are Your Responsibility

    Today is June 20th, the seventh day in the Fire/EMS Safety, Health and Survival Week. This is the culminating day of what hopefully has been an opportune week to dedicate time and energies to focus on the mission critical and life sustaining functional areas of our fire and EMS profession.

    The theme this year was Protect Yourself: Your Safety, Health and Survival Are Your Responsibility and encouraged chiefs and fire/EMS personnel to focus on what they personally could do to manage risk and enhance their health and safety. This year’s theme reflected the need for personal responsibility and accountability within a strong safety culture.

    There were recommended activities and materials provide that would incorporate four key areas where standard operating procedures, policies and initiatives—along with the training and enforcement that support them—can limit fire/EMS personnel’s risk of injury or death.

    These focus areas consisted of;

    Safety: Emergency Driving (enough is enough—end senseless death)
    • Lower speeds—stop racing to the scene. Drive safely and arrive alive to help others.
    • Utilize seat belts—never drive or ride without them.
    • Stop at every intersection—look in all directions and then proceed in a safe manner.

    Health: Fire Fighter Heart Disease and Cancer Education and Prevention
    • Don’t smoke or use tobacco products.
    • Get active.• Eat a heart-healthy diet.
    • Maintain a healthy weight.
    • Get regular health screenings.

    Survival: Structural Size-Up and Situational Awareness
    • Keep apprised of different types of building materials and construction used in your community.
    • Develop a comprehensive size-up checklist.
    • Always complete a 360° walk of the structure to collect valuable, operational decision-making information.
    • Learn the practice of reading smoke.
    • Be familiar with the accepted rules of engagement.
    • Learn your accountability system and use it.
    • Master your tools and equipment.
    • Remain calm and concentrate.

    Chiefs: Be the Leader in Safety
    • Become personally engaged in safety and make it part of your strategic vision for the department.
    • Be willing to make the tough decisions regarding safety policies and practices and their implementation.
    • Hold members of the organization accountable for their safety and the safety of those with whom they work.
    • Ensure that resources are available to accomplish activities safely and effectively.

    The IAFC and IAFF encouraged all fire/EMS departments to devote the period of this week to;
    • review safety policies,
    • evaluating the progress of existing initiatives and
    • discussing health and fitness.
    • Fire/EMS departments were encouraged to make a concerted effort during the week to correct safety deficiencies and
    • provide training as needed.

    The Consciences Observer or Activist
    So the operative question this Saturday is this: What did you do, participate in, contribute, join in, share, lead, promote, instruct, present, facilitate, help, assist, aid, or neglect, disregard, undermine, abuse, challenge, demoralize, undercut, damage, torpedo, circumvent, or avoid?

    A considerable and tangible effort was made by most organizations, departments and staff I had the opportunity to talk to around the nation this week. It was clearly evident that a majority of online fire service trade magazines, journals, services, blogs and eMedia, social + networking communities also dedicated editorial attention and perspectives towards the themes and focus of Fire/EMS Safety, Health and Survival week.

    With that being said, it also doesn’t take long to also see what I alluded to in the, “what did you do this week question?”. Many organizations, personnel and leaders specifically preferred, NOT to participate or did so under a thin veil of apparent involvement. To think otherwise, would be naive and ignorant. For let us not forget, these are the times in which the culture of suppression is contending with the culture of safety…..But, I digress; we’ll leave that alone for the time being.

    Were you an active participant, engaged and contributing or were you the consciences observer, passively or aggressively sitting on the sidelines of the apparatus floor? Campaigner or militant; advocate or protester? Where do you stand?I began this discussion today with one distinct, poignant contemplation and value; Do YOU have the Courage to be Safe? The resonating theme that challenged all of us and carried the banner of the week was; Protect Yourself: Your Safety, Health and Survival Are Your Responsibility. YOUR Responsibility-Not someone else’s, but your; responsibility, task, job, duty, charge, accountability conscientiousness, and obligation.

    Taking it to the Streets
    The adage that the fire service has more recently adopted states; “There are no “routine calls”; referring to the safety consciousness that all responding companies should endeavor to consider when responding to an incident

    .• We have a tendency to treat a lot of things as equal and very routine based upon the periodicity and frequency of the alarm type and the typical, inconsequential nature of the incident outcome or the commonality of the fire and suppression efforts that routinely are employed by our operating companies.
    • We seem to do a lot of things at times out of common practice and repetition, you know; “We’ve always done it that way….” syndrome.
    • There’s a resonating theme that is making its way around the fire service dealing with an apparent “culture of extinguishment” and the suggested and inaccurately described “diametrically opposing” fire service safety culture promoted by those on the “Dark Side”
    • There’s the daily experience, expectations, and our comfort zone;
    • We’re pretty good at what we do-Regularly….
    • We develop profound habits and methods…
    • We treat a lot of things as equal in many respects…

    • We’ve grown accustomed to certain operational modes..
    • We don’t really think anything is going to happen to us, certainly nothing so adverse that I don’t go home after the call.
    • Nothing is going to happen to YOU; it happens to someone else….
    • BUT to everyone else-YOU are the other Guy!

    On any give day, at any give alarm, the dynamics around us at times may be in or out of our direct control. We may not be able to see what the cards have in store for us, BUT we must ensure we use every fragment of training, fortitude, knowled
    ge, skills, courage, bravery, insights, luck and sometimes (other divine) intervention to get us through. We must have the fortitude and courage to be both safety conscious and measured in the performance of our sworn duties while maintaining the appropriate balance of risk and bravery.

    The demands and requirements of modern firefighting will continue to require the placement of personnel within situations and buildings that carry risk, uncertainty and inherent danger. As a result, risk management must become fluid and integrate all personnel. We must manage dynamic risks with a balanced approach of effective assessment, analysis and probability within command decision making that results in safety conscious strategies and tactics.

    Stop and reflect today, where do you stand? What are your true beliefs and convictions in regards to the developing safety culture that is being forged and institutionalized within our fire service? Are your professing one thing, but implementing or allowing another circumstance?

    Think about the following and attach you own significance or connotation;

    The Courage to Be Safe SM Program
    Firefighters must have the courage to face a multitude of risks in order to save lives and protect their communities. Their courage allows them to willingly risk their own lives so that others can be saved. A different type of courage is required to stay safe in potentially dangerous situations, avoiding needless risks and tragic consequences.

    This provocative and moving presentation is designed to change the culture of accepting the loss of firefighters as a normal occurrence. Building on the untold story of LODD survivors, it reveals how family members must live with the consequences of a firefighter death and provides a focus on the need for firefighters and officers to change fundamental attitudes and behaviors in order to prevent line of duty deaths. The central theme promotes the courage to do the right thing in order to protect yourself and other firefighters and ensure that “Everyone Goes Home” at the end of the day.

    Courage to Be SafeSM consists of a PowerPoint presentation on a CD, as well as an instructor guide and sample handout materials. The CD includes embedded video clips of fire service leaders and excerpts of the emotional presentation made at FDIC-2005 by Reverend Bevon Smith, father of Paul Smith, a firefighter who in died in the line of duty in 1989.

    The Safety Through Leadership Program
    In the summer of 2006, National Fallen Firefighters Foundation Executive Director Ron Siarnicki and the Everyone Goes Home® Life Safety Initiatives (LSI) Team attended a meeting in San Diego of the National Wildland Coordinating Group to look at the leadership material the NWCG had developed for the Wildland community. The LSI Team crafted a proposal to develop a course for the structural firefighter based on the NWCG model.

    Shortly thereafter, Lieutenant Tony McDowell, company officers’ section of the Virginia Fire Chiefs Association, contacted the LSI Team seeking help to develop a leadership course that would embody the 16 Firefighter Life Safety Initiatives. The VFCA was invited to collaborate with the LSI Team in the development of a leadership program that would include relevant aspects of the NWCG model and embody the 16 Firefighter Life Safety Initiatives.

    In order to fully understand the NWCG L-380 Leadership sequence, the Everyone Goes Home® Program sent two safety officers, one from Montgomery County Fire Rescue Service and the other from the Virginia Beach Fire Department, to the San Diego Fire Department to participate an L-380 course being conducted there. The two officers reported the experience as life-changing in terms of what they learned and what they came to believe about the linkage between firefighter safety and leadership.

    The pilot was presented in June (2007) at the University of Richmond’s Jepson School of Business, to thirty students from across the state of Virginia and five others who had seen the course advertised on the VFCA website and applied. Over one hundred students applied for the course. By any measure, the Safety Through Leadership was a phenomenal success.

    Over the next year, FLSI course developers carefully evaluated the material from Virginia with an eye toward developing a national curriculum. While most of the format of the VFCA was retained (including material from the L-380 course) new material was developed and added to the national model curriculum.

    A call was put out in the winter of 2008 for thirty students to come to the National Fire Academy. Quickly, all the seats were spoken for, including the return of three who had attended the Virginia pilot. The Train-the-Trainer program commenced on June 24-27, 2008 on the campus of the National Fire Academy in Emmitsburg, Md.

    Safety Through Leadership focuses on the company officer and his or her attitude toward the safety of firefighters within their span of authority.
    It is a role-play based curriculum which begins with several cooperative exercises, including the naming of the fire department under which all activities will proceed for the duration of the training program.

    Five modules regarding safety and leadership were presented to the students, ranging from effective supervisory practices to threat and error management (utilizing the model of crew resource management).The modules are meant to make the program both easy to deliver (one a month, for example) and scalable to the particular audience. A series of excellent video role-plays were developed to reinforce discussions.

    The central goal of the Safety Through Leadership program is to create an emotional impact within company officers so they are encouraged to reflect on their own leadership styles and create changes always with a mindset toward safety.Safety Through Leadership is not meant to replace other leadership programs that may be in place. Rather, it is a value-added approach that requires company officers and other supervisors to set the example of safety in every behavior they model for the firefighters they supervise.

    This means that safety and leadership are linked not only on the fireground (where we would expect it) but also in training and in the long hours of “everyday living” in the station.• The Train-the-Trainer, therefore, spent a good deal of
    time reviewing and reinforcing the 16 Firefighter Life Safety Initiatives so that the trainers will become living ambassadors for Everyone Goes Home®.

    Safety Through Leadership is a program appropriate at both the station-level and to broader audiences such as regional or state training opportunities.
    It can be delivered as a four day academy, or it can be sequentially delivered via the modules.

    • The bottom line is that company officers hold the key to modeling a safety attitude and for doing everything they can to make sure their firefighters go home safe after every call.

    The Safety Through Leadership program is one way this burden can be lightened, and this responsibility wholeheartedly endorsed.

    Protect Yourself: Your Safety, Health and Survival Are Your Responsibility

    I had the opportunity this past April to present a large body of original research in the form of a comprehensive presentation at this year’s Fire Department Instructors Conference (FDIC) in Indianapolis. During the ensuring research phase of the program development, I was immersed in some research and in reviewing a number of case studies and line of duty death reports, while furthering my concepts related to a new methodology on fireground operations and command risk management that I call Five Star Command TM .

    This process and review encouraged me think a lot about the manner in which the modern fireground is managed, the way the current generation of firefighters and officers see themselves, and the manner in which strategic and tactical battle (Incident Action Plans-IAP) are developed and executed. It also prompted me to begin thinking about what, we as firefighters are thinking when we find ourselves in the center of a “good” rockin’ job….you know what I mean, the kind of interior firefighting that we’ve come to define as the essence of being a firefighter, but are attempting to redefine.

    I have been giving a lot of thought over the past couple of months to the continuing challenges and issues surrounding firefighter LODD and the issues of dynamic risk assessment, command decision-making and company level accountability. My recent lecture at the South Carolina Fire Academy’s annual Firefighter Safety and Health Conference provided an enlighten forum with a wealth of critical thinking and dialog amongst the attendees on firefighter safety, command risk assessment and tactical accountability related to opinions on the emerging new model of modern fire suppression strategies and tactics.

    I went further back in time and began to visualize and think about the manner in which I functioned as a company officer when I first got promoted and the kinds of things we used to do; when we were young and both naïve to the true risks of fireground operations and filled with a sense of fireground invincibility. I know, I placed myself or found my company in positions and places of greater risk, “back in the day”, for the sake of getting more nozzle time in a well involved structure fire, or extended our stay-times in hostile places that were not safe or acceptable by today’s standards. WE, were lucky.Any one of us could have then or even in the present day, find ourselves in an instant, in the wrong place, operating under the wrong plan for all the wrong reasons. We looked for ways to increase our “playtime” for the pleasure, enjoyment, adrenaline rush, exuberance and at times euphoric pleasure doing what we do best; and that was fighting fires.

    To think that this is not happening in today’s fire service would be absurd and illogical. If we look at the ways many departments, companies or personnel are operating on the fireground during structural fire operations and the places we are assigning and directing them to operate within, we would be asking ourselves, WHY?There are tremendous national, state and locally efforts and initiatives directed at enhancing firefighter safety, reducing firefighter line of duty deaths and injury rates, on effective command management, skill development, competencies and cultural changes to improve and enhance the fire service. But it all has to start with the basic unit of operation; the Company, the Officer and personnel.

    Today’s incident scene and structural fires are unlike those in past decades and will continue to challenge us operationally when confronted with structural fire engagement and combat operations. Operationally, We need to be doing the right thing, for the right reason in the right place to increase our safety and incident survivability.

    We need to stop “entertaining” ourselves, the job is dangerous, it has risks, we are not invincible, and we can die; at any alarm, in any fire, at anytime for any number of reasons…..Let me leave you with some new thoughts and concepts related to operational safety and the definitions that I’ve come to develop that may support apparent or contributing causes to many of the fire service’s undesired events or incidents. Think about the definitions; think about how they apply to you, your company or your operations; past, present or future.

    MODERN ENVIRONMENT
    The built-environments that form and shape our response districts and communities pose unique challenges to the day-to-day responses of fire departments and their subsequent operations during combat structural fire engagement. With the variety of occupancies and building characteristics present, there are definable degrees of risk potential with recognizable strategic and tactical measures that must be taken.

    Although each occupancy type presents variables that dictate how a particular incident is handled, most company operations evolve from basic strategic and tactical principles rooted in past performance and operations at similar structures. This basis is based upon Predictability of Performance.

    • Modern building constru
    ction is no longer predicable

    • Command & company officer technical knowledge may be diminished or deficient
    • Technological Advancements in construction and materials have exceeded conventional fire suppression practices
    • Some fire suppression tactics are faulted or inappropriate, requiring innovative models and methods.
    • Fire Dynamics and Fire Behavior is not considered during fireground size-up and assessment
    • Risk Management is either not practiced or willfully ignored during most incident operations
    • Some departments or officers show and indifference to safety and risk management
    • Command & Company Officer dereliction
    • Nothing is going to happen to me (us)

    360 DEGREES OF SEPARATION
    The fireground often has competing or conflicting incident priorities, demands or distractions before a complete appreciation of all mission critical or essentialinformation and data has been obtained. The effective assessment of the incident scene is much more than the three-sided size-up methodology of past fireground practices. In fact the term size-up doesn’t align with the newest directions in firefighter safety and incident command management.

    The 360 degree assessment has become the generally accepted standard from which risk assessment is performed and incident action plans derived. The fact that many LODD case studies and reports repeatedly indicate the lack of an effective 360 degree assessment of the incident scene where structural fire engagement is being initiated was a contributing factor or may have contributed to a different incident outcome.Think about the effectiveness and value that the 360 ◦ Degree assessment brings to the development of an effective and valid incident action plan and the tactics that are driven by those identified and assumed assessment indicators. The question is: Are you conducting a 360 upon arrival, and if not WHY?

    REMEMBER
    All command and supervisory personal and operating companies must be able to recognize and appreciate the risks which are present at an incident in order to carry out an effective dynamic risk assessment. The 360 Degree assessment is a mission critical element for effective and safety incident operations.Don’t for a moment think, “it takes too long to perform” or that you don’t have time to conduct, especially from a company officer perspective when you’re deploying and initiating tactical assignments. That extra minute to conduct a “three-sixty” may make all the difference in the world…..There may be three hundred and sixty degrees of safety margin that separate you and your company between injury or death….think about it.

    Situational Awareness and Risk Assessment
    Situation Awareness related to Building Construction, Command Risk Management and Firefighter Safety is another mission critical element. Situation Awareness (SA) is the perception of environmental elements within a volume of time and space, the comprehension of their meaning, and the projection of their status in the near future. It is also a field of study concerned with perception of the environment critical to decision-makers in complex, dynamic situations and incidents. Both the 2006 and 2007 Firefighter Near-Miss Reporting System Annual Reports identified a lack of situational awareness as the highest contributing factor to near misses reported.• Situation Awareness involves being aware of what is happening around you at an incident scene to understand how information, events, and your own actions will impact operational goals and incident objectives, both now and in the near future.

    • Lacking SA or having inadequate SA has been identified as one of the primary factors in accidents attributed to human error.
    • Situation Awareness becomes especially important in the structural fire suppression and firefighter domains where the information flow can be quite high and poor decisions can lead to serious consequences.
    • Dynamic Risk Assessment is commonly used to describe a process of risk assessment being carried out in a changing or evolving environment, where what is being assessed is developing as the process itself is being undertaken.
    • This is further problematical for the Incident Commander when confronted with competing or conflicting incident priorities, demands or distractions before a complete appreciation of all mission critical or essential information and data has been obtained.
    • The dynamic management of risk is all about effective, informed and decisive decision making during all phases of an incident at a structural fire.

    The integration of Situational Awareness and Dynamic Risk Assessment related to the building and occupancy is a mission critical element in managing structural fires and in the strategic command management and company level tactical operations as we go forward into the next decade.

    • Traditional phased incident scene size-up and monitoring is antiquated and no longer appropriate or applicable to modern fire service operations.
    • Situational awareness is a combination of attitudes, previously learned knowledge and new information gained from the incident scene and environment that enables the strategic commanders, decision-makers and tactical companies to gather the information they need to make effective decisions that will keep their firefighters and resources out of harm’s way, reducing the likelihood of adverse or detrimental effects.

    Command and company officers and firefighters MUST understand the building, the occupancy features and the inherent impact of fire within and on the structure, AND be able to identify, communicate and take actions necessary to support the incident action and battle plans, mitigate incident conditions and provide for continuous safety protection to themselves, their team, their company and the entire alarm assignment operating at the incident scene.

    Everyone on the incident scene MUST stay alert to changing conditions, obvious or latent conditions or escalating factors that require prompt identification, comprehension and appropriate implementation of actions. To the Incident Commander, fire officer or firefighter, knowing what’s going on around you, in and around the building structure and understanding the consequences of building, construction, assembly, fire load and fire development and growth is mission critical to incident stabilization and mitigation and profoundly crucial in terms of personnel safety. Maintain a three-sixty sphere of observation and awareness at all times.

    Fire/EMS Safety, Health and Survival Week: Day Five: “From Waldbaum’s to Hackensack- Worcester to Charleston; Legacies for Operational Safety”

    Today is June 18th, the fifth day in the Fire/EMS Safety, Health and Survival Week. Today also commemorates the anniversary of the Sofa Superstore in Charleston, South Carolina, in which nine firefighters lost their lives while engaged in aggressive interior operations at a commercial building, occupied and operating as a furniture store and warehouse.

    On the evening of June 18, 2007, units from the Charleston Fire Department responded to a fire at the Sofa Super Store, a large retail furniture outlet in the West Ashley district of the city. Within less than 40 minutes, the fire claimed the lives of nine firefighters and changed the lives of countless others. The incident galvanized the nation’s fire service and to this day, continues to generate commentary and observations within a wide latitude of functional areas.

    I started this article with the title “From Waldbaum’s to Hackensack- Worcester to Charleston; Legacies for Operational Safety”. I still find it surprising during my travels around the country lecturing and presenting programs on building construction, that when the audience was asked, “What do the Walbaum’s Fire and Hackensack fire share in common?”, the response typically were blank stares. The more seasoned and experienced veterans (translation; Older firefighters) when present, were able to convey some information on the subject. But yet, the true essence of the basic incident particulars and the lessons learned fail to be fully conveyed. We’re not remembering the past!

    I recently spoke about History Repeating Events (HRE), and the common themes related to LODD. Events that resonate with common issues, apparent and contributing causes and operational factors that share legacy issues that the fire service fails to identify, relate to and implement. In other words, we fail a times to learn from the past, or we make a deliberate choice to ignore those lessons due to other internal or external influences, pressures, authority, beliefs, values or viewpoints. We make choices and we determine our direction, path and destiny.

    When you look over these LODD events over the years (NIOSH, NFPA, USFA Reports), it doesn’t take long to identify that many LODD events share similarities, and that specific incident events, deficiencies, outcomes and recommendations are identical in every way, except for the fire department name and geographical location. In other words, we have History Repeating Events (HRE).

    What have we learned from the past? What is it that we’re passing down to each incoming recruit class and probationary firefighter? What are Company and Commanding Officers recalling and considering in their dynamic risk assessment, size-up and decision-making (IAP) process when looking at a particular building, occupancy and fire? Are mission critical operational elements & HRE factors being recollected? (Naturalistic/ Recognition-Prime Decision-making).

    Are the fire service legacies of the past and the lessons learned from those incidents and the sacrifices that were made transcending time? Or are they lost in the immediacy of day to day challenges, issues and operations. Or are these events, lessons and operations issues dismissed and disregarded as a result of their “time and place” not being relevant to “today’s” operations and modern fire service advancements.

    The reality is, we, the present generation of veteran firefighters and officers at times neglect or fail to recognize the importance of passing along the lessons of our life’s journey through our fire service careers, the events of our day and the profound tough lessons and sacrifices learned the hard way. We sometimes need a receptive, sympathetic and compassionate audience that is willing to listen, hear and comprehend the messages conveyed. There needs to be a high degree of empathy related to these past History Repeating Events. For each event, each and every line of duty death has a message and a Legacy of Operational Safety.

    Throughout the past thirty-two years (1977-2009), a total of 3998 firefighters have lost their lives in the course and conduct of their duties as firefighters and officers within the fire service. Although there are numerous LODD fire incidents and events that could be discussed, all distinguished and exemplified by heroism, nobility, cause and fortitude. There are four that stand out when related to the lessons learned and the significance and impact each LODD incident had at the time to the national fire service.

    Make the time to research, learn and understand the factors of these events, the lessons and opportunities that are borne from each and how they relate to the theme, message and initiatives that make up Fire/EMS Safety, Health and Survival Week and beyond. Learn and Educate Yourself. Protect Yourself: Your Safety, Health and Survival Are Your Responsibility.

    Each of these incidents also have significance as they relate to the building, occupancy, use, construction features, inherent structural systems, fire behavior and fire dynamics; coupled with interrelated elements of strategic and tactical fire suppression operations and incident management .     The Waldbaum’s Supermarket Fire: Brooklyn, New York August 3, 1978
    Six LODD

    Six FDNY firefighters died at this fire when the wood bowstring truss roof collapsed, 34 were injured. The fire started at 8:40 hrs. in Waldbaum’s Supermarket, Ave. Y and Ocean Ave., Sheepshead Bay, Brooklyn, NY. Nearly 23 electricians, plumbers etc were in he process of renovating the building, while it was still open and operating when the fire started in the mezzanine area. An All hands was transmitted at 08:49 hrs., the 2nd alarm at 09:02 hrs. Shortly after 09:20 hrs., with 20 firefighters on the roof a crackling sound was heard and the center portion of the bow string trussed roof fell into the smoke and flames. A total of 12 firefighters fell into the inferno, six were rescued, six were died in the line of duty.

    Five fire fighters from the Hackensack, New Jersey Fire Department were killed in the line-of duty while they were engaged in interior fire suppression efforts at an automobile dealership when portions of the building’s wood bowstring truss roof collapsed.

    As a result of this incident passage of a NJ State law mandating the clear demarcation of truss roofs and other structural hazards with warning signs (placards) on building with truss roofs was.     The Worcester Cold Storage and Warehouse Fire: Worcester, Massachusetts, December 3, 1999

    On December 3, 1999, the vacant, six-story Worcester Cold Storage and Warehouse Co. building in Worcester, Massachusetts, was set ablaze by two homeless people knocking a lighted candle into a pile of ragged clothes. The Worcester Fire Department responded at 6:13 p.m. to Box Alarm 1438. The Rescue 1 team of Firefighter Paul Brotherton and Firefighter Jerry Lucey entered the building searching for occupants. Fire conditions worsened in the building at an alarmingly unexpected rate. Paul and Jerry, on the fifth floor, became disoriented in the smoke-filled building. Lost, and running low on air, they called for help. Several teams began searching for the lost fire fighters.

    Two teams reaching the fifth floor also found themselves disoriented in the smoke and trapped by the maze of interior walls — Lieutenant Tom Spencer and Firefighter Tim Jackson from Ladder 2, and Firefighter Jay Lyons and Firefighter Joe McGuirk from Engine 3. Though many more brave fire fighters attempted to locate their missing brothers, their efforts proved futile. Their deaths marked the worst loss of fire fighters’ lives in more than 20 years in a building fire in America, and the third worst fire in Massachusetts’ history. Six days after they died, a memorial service drew 30,000 fire fighters and 10,000 civilians in what was believed to have been the largest such service for fire fighters killed on duty.

    Commemorate and Remembrance

    The executive summary of the FIREFIGHTER FATALITY INVESTIGATIVE REPORT Sofa Super Store Fire, Phase II Report issued MAY 15, 2008 provided critical insights into the apparent and contributing causes that culminated in the event. The Sofa Super Store was a large property that incorporated a very significant potential for a major fire to occur. It’s appropriate at this time to revisit those key factors described within the report in order for provide the opportunity for departments or agencies to recognize or identify similar gaps that exist, and take the necessary corrective actions. These gaps may be precursors to potentially significant or serious future events and extend in operational, training, administrative, managerial, construction, prevention and regulatory and codes.

    · The fire risk factors associated with the Sofa Super Store exceeded the limits prescribed by the applicable building and fire codes. An automatic sprinkler system should have been installed to reduce the level of fire risk or the buildings should have been divided into manageable fire compartments by a system of fire walls.
    · If a sprinkler system had been installed, the fire probably would likely have been controlled within the loading dock area.
    · If effective fire walls had been provided, the fire probably would not have spread beyond the loading dock.
    · The highly flammable characteristics of the materials that were stored in the loading dock and throughout the premises provided an ample supply of fuel and caused the fire to spread rapidly. The burning contents released copious quantities of heat and toxic smoke.
    · Significant quantities of flammable and combustible liquids that were stored in the loading dock likely contributed to the severity and rapid spread of the fire.
    · The fire had extended to the loading dock when firefighters arrived.
    · Charleston Fire Department members attempted to fight the fire by initiating an offensive interior attack into the loading dock.
    · The offensive attack was launched from two directions. One attack line entered the loading dock from the exterior, while a second line was stretched through the showrooms and into the loading dock.
    · The offensive attack failed to control the fire. The fire extended into adjoining areas on three sides of the loading dock.
    · At least 16 firefighters, who were operating deep inside the showrooms, became enveloped in heavy smoke.
    · Conditions inside the showrooms became critical as the fire began to involve this part of the building. Several firefighters became disoriented and were running short of air. Radio messages requesting assistance were not heard.
    · Seven firefighters managed to find their way out of the showrooms. The nine deceased firefighters were unable to find their way out as the fire spread rapidly from the rear of the building to the front.
    · The size and layout of the building, inadequate exits, and the highly flammable nature of the contents likely contributed to the inability of the lost firefighters to escape from the building. Rescue efforts were attempted when the situation inside the showrooms was recognized. In spite of valiant efforts, it was too late to save the missing firefighters before the store became fully involved in flames.

    The analysis of operations conducted by the Charleston Fire Department includes the following observations and findings:

    · Fire fighting operations at the Sofa Super Store did not comply with Federal occupational safety and health regulations, recommended safety standards, or accepted fire service practices.
    · The Charleston Fire Department failed to provide adequate direction, supervision, and coordination over the operations that were c
    onducted.
    · The documented duties and responsibilities of an Incident Commander were not performed and risk management guidelines were not adequately applied to the situation.
    · The culture of the Charleston Fire Department promoted aggressive offensive tactics that exposed firefighters to excessive and avoidable risks and failed to apply basic firefighter safety practices.
    · Insufficient training, inadequate staffing, obsolete equipment and outdated tactics all contributed to an ineffective effort to control the fire with offensive tactics during the early stages of the incident.
    · The Charleston Fire Department continued to apply offensive tactics after the situation had evolved to a point where risk management guidelines called for defensive strategy.
    · Factors that should have caused firefighters to be removed from interior tactical (offensive) positions were not recognized.
    · There was a lack of accountability for the location and function of firefighters who were operating inside the building. The Charleston Fire Department did not have appropriate Mayday procedures to be followed by firefighters in distress, for dispatchers, or for command officers on the scene.

    All of the listed factors and many others were analyzed and discussed in detail within the body of the issued report. If you haven’t found the time or reason to read the report, do so; it would make for a good task activity for Safety Week. The report document presented the dedicated and conscientious efforts of the review team to honor the nine fallen firefighters by making every possible effort to learn from their sacrifice. The operative question is this; “What factors or attributes are comparable to situations or conditions that presently exist within your Department, Organization or community? What are you going to proactively do to address these issues or conditions in a timely manner?

    Understanding the Building Profile and Risk
    The Sofa Super Store occupied a complex of interconnected structures that had been constructed in several phases. The showroom building, facing Savannah Highway, was actually an assembly of three separate structures. The front wall was a façade, with a parapet extending above the roof line, creating the appearance of one large building when viewed from Savannah Highway. (Refer to the Report for diagrams, plans and photographs)
    · The front wall, including the parapet, was approximately 23 feet tall, while the roof behind the parapet varied from 12 to 14 feet above grade.
    · The main showroom was originally constructed as a grocery store, probably during the 1950s or 60s. The original building was approximately 125 feet in width and 130 feet deep, with a rectangular extension in the southwest corner (right-rear facing the building from Savannah Highway).
    · The front wall was brick construction with large storefront windows, while the side and rear walls were constructed of concrete block.
    · The original structure had a flat metal deck roof, supported by lightweight steel bar joists (trusses), spanning from east to west across the store. The side walls supported the ends of the bar joists, while two rows of steel beams and columns provided intermediate support.
    · A suspended ceiling was installed below the roof trusses.

    After the property was converted to a furniture store, two pre-engineered metal buildings were added-on to the original structure to expand the showroom area. Each showroom addition was approximately 60 feet in width and 120 feet deep. The first showroom addition was constructed on the west side of the original building in 1994 and the second was added on the east side in 1995. (The add-on structures are referred to as the east and west showrooms in this report, while the original structure is identified as the main showroom.) Six large openings in the concrete block side walls, three on each side of the original building, provided connections between the showroom areas; their combined floor area was in excess of 31,000 square feet. An additional pre-engineered metal structure was erected at the rear of the property in 1996 to serve as a warehouse. This structure was approximately 120 feet wide by 130 feet deep and 29 feet tall. Furniture was stored on steel racks, 20 feet in height, inside the warehouse.

    Fire/EMS Safety Week Initiative
    The Monroe County (NY) Fire Marshals and Inspectors Association (MCFMIA) conceptualized an idea following a presentation on the Charleston Fire at the National Fire Academy during the recent PARADE (Prevention Advocacy Resource and Data Exchange) Conference, to commemorate and honor the fallen Firefighters and to prevent a similar tragedy from occurring and align with the anniversary of the Sofa Superstore. The suggested actions for all fire departments to perform were this:

    · Inspect – Between June 9 and June 18th we ask all local municipalities to conduct a fire safety inspection of all furniture stores in their jurisdictions
    · Pre-Plan – Between now and June 18th we ask all fire departments to update or complete a pre-plan on all the furniture stores in their first due areas. Share the pre-plan with neighboring departments, companies or stations that would respond on a fire with you.
    · Get Out and Examine, Inspect, Inquire and Report your findings.

    Going Forward: The Structural Anatomy of Building Construction

    “If you don’t fully understand how a building truly performs or reacts under fire conditions and the variables that can influence its stability and degradation, movement of fire and products of combustion and the resource requirements for fire suppression in terms of staffing, apparatus and required fire flows, then you will be functioning and operating in a reactionary manner.”

    “This places higher risk to your personnel and lessens the likelihood for effective, efficient and safe operations. You’re just not doing your job effectively and you’re at RISK. These risks can equate into insurmountable operational challenges and could lead to adverse incident outcomes. Someone could get hurt, someone could die, it’s that simple, it’s that obvious. “
    Risk Based Response Assignments
    The buildings, structures and occupancies that comprise typical response districts pose unique and consistent challenges during structural fire attac
    k. The variety of occupancies and building characteristics establish varying degrees of risk potential, with defined and recognizable strategic and tactical measures to be taken-sometimes uniquely to each occupancy type. Although each occupancy type presents variables that dictate how a particular incident is handled, most company operations evolve from basic principles rooted in past performance and operations at similar structures. This is based on what I define as; “predictability of performance.”

    When we look at various buildings and occupancies, past operational experiences; those that were successful, and those that were not, give us experiences that define and determine how we access, react and expect similar structures and occupancies to perform at a given alarm in the future. Naturalistic (or recognition-primed) decision-making forms much of this basis. We predicate certain expectations that fire will travel in a defined (predictable) manner that fire will hold within a room and compartment for a given duration of time, that the fire load and related fire flows required will be appropriate for an expected size and severity of fire encountered within a given building, occupancy, structural system.

    We used to know with a measured degree of predictability, how our buildings would perform, react and fail under most fire conditions. This is what our years of fireground experience provided us, and how we ultimately would predict, assess, plan and implement our incident action plans and ultimately deploy our companies-based upon the predictable performance expected. Conventional Construction Structures (CCS) had this “predictably of performance.” You know, that typical residential structure, the 2-1/2 story wood frame, the three story brick and joist type III occupancy, the four story frame multiple occupancy, etc., etc. Unlike Engineered System Structures (ESS) whose predictability is rooted in the fact that they are unpredictable.

    The emerging fire service issues affecting buildings, occupancies and structural systems related to ESS is only beginning to take hold a prominent role and level of significance that is long overdue. The fire service has been dealing with the operational issues and line-of-duty deaths related to ESS since the 1980s and now in 2009, we’re finally raising these ESS issues to a dialog point that is influencing firefighter safety, survival and operations.     The fire service is beginning to fully recognize the merits in adjusting, altering, and changing our strategic and tactical ways of doing business in the streets. It’s becoming self evident in the fire service that it’s no longer acceptable to think that ESS buildings and occupancies will perform in the same manner as CCS buildings and occupancies and that tactics deployed in both CCS and ESS buildings and occupancies will react under similar strategic and tactical plans and tasks. These unique and inherent factors within the ESS profiles must give us a new standard for operational deployment; strategies and tactics that are defined by the risk profile of the building, its engineered structural systems, materials and methods of construction and the fire loading present.

    Considerations for changing fire flow rates, the sizing of hose line and the adequacies for fire flow demand and application rates, staffing needs for safe operations, considerations for defensive positioning and defensive operating postures must be considered, and it warrants repeating again; Reckless-Aggressive firefighting must be redefined in the built environment and associated with goal oriented tactical operations that are defined by risk assessed and analyzed tasks that are executed under battle plans that promote the best in safety practices and survivability within know hostile structural fire environment- with determined, effective and proactive firefighting.
    Risk-Preferring and Self-indulging Firefighting
    Don’t mistake determined, effective and proactive firefighting with that of reckless, baseless and risk-preferring and self-indulging firefighting. There is a difference, a big difference! When we address relationships of Building Construction, Command Risk Management and FireFighter Safety with the occupancy and structural environment, all personnel, regardless of rank, need to equate the occupancy risk with strategic and tactical incident action plans. These safely compliment the identified firefighting operation risk, with the projected building risk profile and interface appropriate behavioral characteristics in the task level firefighting activities. Again, equating building, occupancy risk profiles with determined, effective and proactive firefighting.
    The traditional attitudes and beliefs of equating aggressive firefighting operations in all occupancy types coupled with the correlating, established and pragmatic operational strategies and tactics MUST not only be questioned, they need to be adjusted and modified; risk assessment, risk-benefit analysis, safety and survivability profiling, operational value and firefighter injury and LODD reduction must be further institutionalized to become a recognized part of modern firefighting operations.
    It’s no longer just brute force and sheer physical determination that define structural fire suppression operations. Aggressive firefighting must be redefined and aligned to the built environment and associated with goal oriented tactical operations that are defined by risk assessed and analyzed tasks that are executed under battle plans that promote the best in safety practices and survivability within know hostile structural fire environments. Consider the following definitions as they relate to defining structural combat fire suppression operations.
    Aggressive and Measured Approach.
    Aggressive: Assertive, bold, and energetic, forceful, determined, confident, marked by driving forceful energy or initiative, marked by combative readiness, assured, direct, dominate…
    Measured: Calculated; deliberate, careful; restrained, think, considered, confident, alternatives, reasoned actions, in control, self assured, calm…
    You be the judge as to what should be appropriately defining interior fire suppression operations.
    It’s all about understanding the building-occupancy relationships and integrating; construction, occupancies, fire dynamics and fire behavior, risk, analysis, the art and science of firefighting, safety conscious work environment concepts and effective and well-informed incident command management. This is what it’s going to take to truly provide a means for “everyone to go home”.
    Occupancy Risk not Occupancy Type Many of today’s incident commanders, company officers and firefighters lack the clarity of understanding and comprehension that correlate to the inherent characteristics of today’s buildings, construction and occupancies. We assume that the routiness of our operations and incident responses equates with predictability and diminished risk to our firefighting personnel.
    Our current generation of buildings, construction and occupancies are not as predicable as past conventional construction, therefore risk assessment, strategies and tactics must change to address these new rules of structural fire engagement. You need to gain the knowledge and insights and to change and adjust your operating profile in order to safe guard your companies, personnel and team compositions. Again strategic firefighting operations; Strategies and tactics must be based on occupancy risk not occupancy type.
    With this being stated, another primary consider
    ation that must be deliberated and changed as it relates to firefighting and the built environment is the long held fire service tradition and practice of Structural Fire Alarm Response (resources) Assignments being based upon the Occupancy Type. Sending the two Engine Companies and one Truck Company assignment with a Battalion Chief and a RIT team to a reported structure fire in an occupied single family residential structure; is not acceptable.
    As I previously stated, the rules for structural fire engagement have changed. Structural Fire Alarm Response (resources) Assignments should be based upon the Risk Profile the occupancy has related to Building construction, systems and projected or determined fire loading. Sending the four Engine Companies, two Truck Companies, a manpower Heavy Rescue Company, two additional Battalion Chiefs, a Safety Officer and support staff assignment with the assigned Battalion Chief on the alarm assignment to a reported structure fire in an occupied single family residential structure, that happens to be 5000 square feet in size with ESS components; IS Acceptable.
    · There is an acute understanding and corollary of technical knowledge and inter reliance on occupancies, construction, strategy, tactics, risk, safety, physics, engineering and fire suppression theory, This is a fact.
    · Previous, historical parameters and Building/Structural Performance always provides a postulated measurement to gauge operational tasks and form the basis for the Incident Action Plan. These parameters must be recognized and integrated
    · There is a need to integrate performance based incident indicators derived from engineering, physics, fire dynamics, historical and statistical basis
    · Basic Size-Up is Antiquated for Firefighting and the Built Environment. – Start Thinking in terms of Dynamic Risk Assessment and Command Risk Management
    · USFA Annual Report on Firefighter Fatalities in the United States “More firefighters using an aggressive interior attack in enclosed structures die more often, in greater numbers, and with greater multiple line-of-duty deaths than those using the same tactical approach in opened structure fires.”
    Situational Awareness and Risk Assessment Situation Awareness related to Building Construction, Command Risk Management and Firefighter Safety is another mission critical element. Situation Awareness (SA) is the perception of environmental elements within a volume of time and space, the comprehension of their meaning, and the projection of their status in the near future. It is also a field of study concerned with perception of the environment critical to decision-makers in complex, dynamic situations and incidents. Both the 2006 and 2007 Firefighter Near-Miss Reporting System Annual Reports identified a lack of situational awareness as the highest contributing factor to near misses reported.
    Situation Awareness involves being aware of what is happening around you at an incident scene to understand how information, events, and your own actions will impact operational goals and incident objectives, both now and in the near future. Lacking SA or having inadequate SA has been identified as one of the primary factors in accidents attributed to human error (Hartel, Smith, & Prince, 1991) (Nullmeyer, Stella, Montijo, & Harden, 2005). Situation Awareness becomes especially important in the structural fire suppression and firefighter domains where the information flow can be quite high and poor decisions can lead to serious consequences.
    Dynamic Risk Assessment is commonly used to describe a process of risk assessment being carried out in a changing or evolving environment, where what is being assessed is developing as the process itself is being undertaken. This is further problematical for the Incident Commander when confronted with competing or conflicting incident priorities, demands or distractions before a complete appreciation of all mission critical or essential information and data has been obtained. The dynamic management of risk is all about effective, informed and decisive decision making during all phases of an incident at a structural fire.
    To the Incident Commander, fire officer or firefighter, knowing what’s going on around you, in and around the building structure and understanding the consequences of building, construction, assembly, fire load and fire development and growth is mission critical to incident stabilization and mitigation and profoundly crucial in terms of personnel safety.
    The integration of Situational Awareness and Dynamic Risk Assessment related to the building and occupancy is a mission critical element in managing structural fires and in the strategic command management and company level tactical operations as we go forward into the next decade. Traditional phased incident scene size-up and monitoring is antiquated and no longer appropriate or applicable to modern fire service operations.
    Situational awareness is a combination of attitudes, previously learned knowledge and new information gained from the incident scene and environment that enables the strategic commanders, decision-makers and tactical companies to gather the information they need to make effective decisions that will keep their firefighters and resources out of harm’s way, reducing the likelihood of adverse or detrimental effects.
    Command and company officers and firefighters MUST understand the building, the occupancy features and the inherent impact of fire within and on the structure, AND be able to identify, communicate and take actions necessary to support the incident action and battle plans, mitigate incident conditions and provide for continuous safety protection to themselves, their team, their company and the entire alarm assignment operating at the incident scene.
    It’s Not about Our Entertainment Value
    When we focus out attention on the interdependent functional domains of Building Construction, Command Risk Management and FireFighter Safety and the essence of combat structural fires; Structural firefighting is what it’s all about, is it not? The reason we have such veneration for firefighting and the fire service and all it entails; has a lot to do with going into burning buildings and fighting fire. We enjoy it tremendously; we have fun at, because of who we are and what we do-as firefighters. But, firefighting has its adverse consequences, with all too familiar costs, in the form of injuries, debilitating accidents and line of duty deaths.
    As a firefighter, to say that we love firefighting would be an understatement, BUT one issue that we need to address is the fact that there are many individual firefighters, companies and organizations that employ fireground operational practices that promote the “enjoyment and entertainment” of working a good job within the occupancy compartment of a structural fire in the building environment. Staying too long in the wrong place, operating tactically in an adverse environment with known hazards that do not have value, other than the enjoyment of nozzle time and operating time in the fire.
    Fire suppression tactics must be adjusted for the rapidly changing methods and materials impacting all forms of building construction, occupancies and structures. The need to redefine the art and science of firefighting is nearly upon us. Some things do stand the test of time, others need to adjust, evolve and change. Not for the sake of change only, but for the emerging and evolving buildings, structures and occupancies being built, developed or renovated in our communities.
    If the fire service can significantly increase proficiencies in building knowledge and equate that to other fundamental operational aspect in structural fire operations, then there would be a direct enhancement to firefighter safety, through injury and LODD reduction. If we understand buildings, occupancies and construction, and balance this
    with our understanding of fire dynamics and orchestrate it with appropriate strategies, tactics and command management, then we made the new safety equation work; Building Knowledge = Firefighter Safety (Bk=F2S).
